Journal Reading (Peplau and the Psychodynamic Nursing)

Remembering Hildegard Peplau

On the 17th of March 1999, HildegardPeplau died at the age of 89, ending a nursing career, which spanned over fifty years. Peplau is often recognised as the ‘mother of psychiatric nursing but her ideas have influenced all fields of nursing. In 1948 Peplau completed her seminal work, ‘Interpersonal Relations in Nursing’. However, it wasn’t published until 1952 because at the time it was too revolutionary for a nurse to publish a book without a medical practitioner as a co-author. This work which has been published in nine languages, has perhaps done more to facilitate the development of nursing from an occupation to a profession than any other. Sills (1998, p.171) described Peplau as “…a woman of uncommon intellect, socialized outside the traditional 1940s model of nursing in the United States, who developed a paradigm of professionalism…. that has permeated every aspect of her long and distinguished career.”

Even if a nurse has never read any of the hundreds of articles or books, which cite and develop Peplau’s ideas, it is likely that she or he, will still have been influenced by them. Peplau suggested that nurses can and do make a difference to people in the context of interpersonal relationships, which have a discernible pattern, predictable phases and during which the nurse assumes roles such as counsellor, leader or teacher.


Some may recognise many of her ideas as being similar to those developed in role theory, cognitive behavioural therapy, symbolic interactionism, or reflecting ‘common sense’. A number of studies suggest that Peplau's theory remains a common frame of reference for most psychiatric nurses (Hirschmann, 1989). Some selected ideas from the works of Peplau (1952, p16) described nursing as "a significant, therapeutic, interpersonal process. It functions co-operatively with other human processes that make health possible for individuals in communities .... Nursing is an educative instrument, a maturing force, that aims to promote forward movement of personality in the direction of creative, constructive, productive, personal and community living". Lack of growth, for whatever reason, implies impaired health in the individual and basic human needs must be met if a healthy state is to be achieved and maintained (Stuart & Sundeen, 1987, p 46).


A number of assumptions are implicit in Peplau's original and latter writings (Forchuk, 1991). For example, the relationship of nurse and patient is influential in the outcome for the patient; People may assume a number of roles and have the capacity for empathy in relationships (Torres, 1986, p 171); People tend to behave in ways which have worked in the past when faced with a crisis (Forchuk, 1991); Anxiety and tension arise from unmet or conflicting needs, and the energy which arises may be harnessed into positive means for defining, understanding and meeting the problem at hand.


The nurse patient relationship is characterised by a number of overlapping phases with a number of therapeutic tasks or goals to be accomplished. During each phase the patient expresses needs which find expression and require intervention in unique ways.


Orientation Phase

The phase of the relationship, when the nurse and patient first meet is known as the orientation phase. This is a time when the patient and nurse come to know each other as people and each other’s expectations and roles are understood. The patient at this time needs to recognise and understand their difficulty and the need for help, be assisted to plan to use the professional services offered, and harness the energy derived from felt needs (Peplau, 1952, p 19). It may be expected that the patient will test limits in order to establish the integrity of the nurse. The tasks of this phase are to build trust, rapport, establish a therapeutic environment, assess the patients strengths and weakness and establish a mode of communication acceptable to both patient and nurse (Shives, 1994, p 91). When the patient can begin to identify problems the relationship progresses to the working phase.


Working Phase

The working phase incorporates the identification and exploitation sub-phases and the relationship may fluctuate back and forth as new problems are identified. During the identification phase trust begins to develop and the patient begins to respond selectively to persons who seem to offer help. The patient begins to identify with the nurse and identify problems, which can be worked on. The meaning behind feelings and behaviour of the nurse and patient are explored. Peplau (1952, p31) states that when a nurse permits patients to express what they feel, and still get all of the nursing that is needed, then patients can undergo illness as an experience that reorients feelings and strengthens positive forces in the personality. The tasks of this phase are to develop clarity about the patient's preconceptions and expectations of nurses and nursing, develop acceptance of each other, explore feelings, identify problems and respond to people who can offer help. In particular the nurse assists in the expression of needs and feelings, assists during stress, shows acceptance and provides information. The nurse and patient may make plans for the future but the implementation of the plan signifies the beginning of the exploitation phase of the working relationship. During the exploitation phase the patient realistically exploits all of the services available to them on the basis of self interest and need (Peplau, 1952, p 37). The nurse assists the patient in their efforts to strike a balance between the needs for dependence and independence. The plan of action is implemented and evaluated. The patient may display a change in manner of communicating, as new skills in interpersonal relationships and problem solving are developed (Forchuk & Brown, 1989, p 32). The nurse continues to assess and assists in meeting new needs as they emerge.


Resolution Phase

The resolution phase involves the gradual freeing from identification with helping persons, and the generation and strengthening of ability to stand alone, eventually leading to the mutual termination of the relationship (Peplau, 1952, p 39). The patient abandons old needs and aspires to new goals. She or he continues to apply new problem solving skills and maintains changes in style of communication and interaction. Resolution includes planning for alternative sources of support, problem prevention, and the patient’s integration of the illness experience.


Roles of the Nurse

The nurse may assume different roles within the relationship. The first role assumed by both the nurse and patient is stranger. This role requires respect and positive interest on the part of the nurse. The nurse may function as a resource person, providing specific answers to questions usually formulated with relation to a larger problem. As teacher the nurse assists the patient as a learner to grow and learn from experience. As leader the nurse may assist the patient as follower in a democratically implemented nursing process (Stuart & Sundeen, 1987, p 46). The nurse may be cast into surrogate roles by patients based on their significant past relationships. Considerable importance is also assigned to the role of the nurse as counsellor which is viewed as helping the patient integrate the facts and feelings associated with an episode of illness into his or her total life experience. Nurses may assume many other roles but in the context of the interpersonal relationship all aim to assist the patient to meet the goals of therapy, need satisfaction and growth (Stuart & Sundeen, 1987, p45).


Communication

Peplau (1952) expanded on a number of concepts drawn from developmental, psychoanalytical and behavioural theory in order to assist the nurse in understanding behaviour. Peplau (1952, p 290) defined communication as an interpersonal process involving the selection of symbols or concepts that go some way towards developing a common understanding. Anxiety is present in all individuals to some degree and is cognitively triggered by real or imagined, internal or external threats to an individuals security (Forchuk, 1991). The ability to empathise with the patient, and self-understanding are necessary requisites of the nurse as therapist. Stuart and Sundeen (1987) describe Peplau's concepts as belonging to a school of thought called the interpersonal view of behavioural deviations which hold that behaviour evolves within the context of interpersonal relations. A common belief within this theory is that by experiencing a healthy relationship with the therapist, the patient can learn to have more satisfying interpersonal relationships in general. Closeness within the relationship is seen as necessary as it builds trust, empathy, enhances self-esteem and fosters growth towards healthy behaviour. The process of therapy involves re-educating the patient in more successful ways of relating.


Nursing’s debt to Peplau

This brief and selective summary cannot do justice to Peplau’s ideas, which have been developed, expanded and put into practice over the last fifty years. It is my hope that more people will continue to access Peplau’s original work, which will highlight that she was primarily a pragmatist rather than an academic theorist. It captures the essence of what nursing is and can become, lending it a timeless quality. Peplau developed knowledge with practical import on learning, anxiety, hallucinations, interpersonal concepts, individual, family and group therapy. She articulated a framework for partnership and recovery, which continue to have significance for contemporary practice.


Peplau developed and taught in the first graduate programme in psychiatric nursing in the United States, assisted in the development of the first graduate programme in Europe and was instrumental in developing the role of ‘nurse specialist’. She was an advisor to the World Health Organisation, a member of the International Council of Nurses, a fellow of the Academy of Sigma Theta Tau, and has been awarded some of nursing highest honours. Few students struggling to complete academic studies would be failed to be impressed by Peplau’s credentials, which include no less than seven doctoral degrees!


Peplau (1987) viewed psychiatric nursing as a viable, complimentary and necessary alternative to psychiatric-medical treatments. She encouraged nurses to be proud of the development of nursing and challenged us to remember that… Somewhere, somehow, at some time in the past, courageous nurses determined these skills, learned them, fought for the right to use them, refined them, and taught them to other nurses. All nurses have an obligation to remember that part of nursing’s past, and to keep their own skills in pace with new opportunities for nursing into the next century. (Peplau, 1989, p.32), Peplau was one such courageous woman who has left us a legacy of practical knowledge to enable the survival and continued development of nursing. We are forever in her debt.

hildegard peplau died at the age of 89 but still her legacy is still alive and her theory lingers in ourselves.we our using her theory not only in our nursing profession but in everyday living.because of her, i learned how to control my emotion.i know in myself that i am a kind of person who easily lose temper. but when i finally know hildegard peplau and her theory,i should know how to control my emotion as well as how to treat well my patient.i also realize that my chosen career is not easy. it requires not just only an intelligent quotient but most importantly it requires our emotional quotient.we must understand the patient's behavior.we must be the symbol of life with them. and in her theory i learned different phase on how i will treat my patient.these phases should not be failed for us to achieve the adequate caring in using the psychodynamic nursing.

After I've read this article about Hildegard Peplau of her Psychodynamic Nursing, i realized that a nurses' nature of being caring and patient are also important in a clinical setting. "ASK" is important in nursing. ASK means A-attitude S-skills K-knowledge. Every pers0n specially nurses should have these 3. A mood of a person is contagious. Nurses should establish a good relationship and rapport in order to have a good communication between the nurse and the patient to heal the patient easily.

After reading this article, I have learned that the four phases generated by Peplau was very useful in our profession. By these different phases we should consider the proper way to communicate with our patient. It is mandatory that the nurse knows how to handle the patient's situation. Verbal and non-verbal communication between the patient and the nurse helps the individual to recover from such illness easily.

Different roles of the nurse can be put on. Like being a stranger, in this role there must be respect and interest on the nurses' part; as a resource person, the nurse must provide specific answers to a patients doubt; a teacher, the nurse must teach the patient to learn from his/her experience; as the leader, the nurse must act as the leader and as the patient as the follower to execute the nursing process; surrogate role, this means that a nurse can be a substitute for the patient's previous relationships but it is only temporary; and the counseling role (which has the greatest importance and emphasis on nursing) the nurse becomes the patient's friend, a family and an adviser.

By this theory of Psychodyanamic Nursing, it really taught me that a good communication helps in the betterment of the relationship.

As I have read this journal, the life of Hildegard Peplau is very inspiring. It really inspires me to become a nurse someday and I will apply her Psychodynamic Nursing so I will leave a good experiances and memories to my patient. This journal further explained the Psychodynamic Nursing of Hildegard Peplau. It states how we can cure the mentally ill person. The job of the nurses are stressfull so they must learn how to manage those stress for them not to affect their duties.

Hildegard Peplau's theory really helps a lot,she reminds nurses not just to focus on recuperating patients but also to have a good communication with them.
It is such a stress whenever you are ill and that you have a terrible nurse at the same time,patient will just look at nurses as stressors.
But its really a good feeling that there is someone who care for you,and it even help to make you get well faster.
They say that her theory was just common sense,but I think it is more than that,those people who don't really love their job, don't know how to apply Peplau's theory.
I just wonder,why is it revolutionary for a nurse to publish a book without nurse practitioner as coauthor,is that they don't have that trust on nurses.

after reading the Journal of Hildegard Peplau, I really understand it. I was amazed for what she have showed for us. she inspires me alot. She gave me a reason to become a nice person and a good nursing student. She also gave me an Idea of being a good explorer and a good listener.

In her theory, She thought us on how to beciome a good nurse, she thought us on how to become a good entertainer to our patient, and of course, she thought us on how to communicate well with our patient.

She gave us an interest to become the kind of person that can make a good relationship with others especiallt with our patient.

in this article about hildegard peplau, i realize and assumed that peplau is such a very intelligent person having a strong personality. understanding one's character and giving assistance to them is not an easy thing to do. it requires a lot of knowledge, courage and experience to formulate a concept on how to cure mental disorders. dealing with stressfull situations can affect one's personality..and i really admire her for making this theory, cause it gives me a lot of ideas on how to care and analyze those people around me based on their actions..

Peplau played an emphasis on the 4 phases of relationship. It can be concluded that if you fail to do the first step, then you’ll make it hard or fail to do the second step.

As you said ma’am, first impression lasts in the nursing profession. So I guess that we, especially on the first day of our coming duty should make it the best. We should start with a smile, then interact with every person in the hospital well. We should be nice to every patient, as well as the other persons around us. Our main target here is to gain the patient’s trust so we can have a good interaction. And gaining trust will give us the confidence that we can handle and manage taking care of a patient properly. We must not forget that we are not simply a nurse rendering care, we are a nurse with 6 roles.

Truly, we are forever in debt of Peplau. We should thank her for viewing psychiatric nursing as an alternative to psychiatric-medical treatments and for encouraging nurses to be proud of the development of nursing

Through this article, I have learned many things from the life story of Peplau and her Psychodynamic theory. It brings a lot of information regarding to the thoery which we can be used in our daily living especially on how to cope up to the relationship between the nurse and the patient.
I can assure that it will really help us especilly those who are taking up the nursing profession. Reading the journal of Hildegard peplau is really amazing and a way of help in giving the best care for our patient. Her theory is really an application in dealing with our patient and handling stress. Her theory is helpful in our education, research, and practice. with her theory we can have a good relationship and interaction with our patient to the betterment of his recovery. Her theory helps a lot in dealing with our patient especially those with mental problem. With her theory, the patient and the nurse create an interaction which is helpful for the betterment of her condition. Her theory is really important in nursings and also in our daily lives

..Hildegard Peplau, a nurse theorist who introduced the therapeutic nurse-patient relationship, started the second wave of changes in the nursing profession, the emphasis given to the dynamics of social interactions between the nurse and the patient was considered little and lacking in answering the health needs of the client.
..As the nurse continually gives care based on a plan made by the team and by the patient, interactions with other health care professions can enhance an atmosphere of cooperation. Students and nurses must understand the processes inside this dynamic relationship. It is very significant that they understand the roles and the phases that the nurse and patient goes through; the communication techniques needed to achieve therapeutic results; and lastly, the elements needed in providing a lasting and a maturing experience for the patient, both sick and well.

The Psychodynamic Theory of Hildegard Peplau helped a great deal in the nursing profession. Through this theory, the nurse- patient relationship is being established to immediately alleviate the patient’s illness. There should be proper communication between them to have more desirable results.

Also I have learned that, in dealing with patients, nurses should be capable of handling the emotions of the patients as well as providing his needs and informing him about his current situation. Peplau’s theory is applicable in a wide range of nursing practice, and also to our daily living.

While I am reading the article of Hildegard Peplau I was really inspire to her theory the Psycodynamic Nursing because I realize how her theory important to the mentally disorder patient and to her nurse.Her theory has become an essential part in todays advance area of nursing,because of this mental disorder are easily interpreted with valid scientific principles it has contributed greatly to the study of nursing.In this journal I learned the nurse-patient relationship and the phases of nursing that helps me a lot in my study.

Like I have said before the theory of Hildegard Peplau opens many opportunities for the nursing community to improve their qualities of care especially for the patient and his family. Hildegard was a very intelligent person, she had many ideas in her mind that i personally think is very contagious in the sense that it can relate to your own sets of perception in the nursing field. Generally, as I have said a dozen times, she is an outstanding individual and a nursing theorist.

What is remarkble regarding Hildegard Peplau is her sympathy for the people who are mentally-ill. Usually, people don't pay attention to those who are psychotic, thinking that they are nothing of importance not considering that they are still humans like us.

Peplau was very hardworking in enduring such kind of nursing intervention done for the mentally-ill and for the people who needs an emotional care.

Because of her, the mutualism between the nurse and the patient begun and this fact leads evidently for the betterment of the patient as well as the nursing care.

it's really so amazing thinking how intellectually-equipped and versatile the nurses are...

As I`ve read the journal about Peplau I`ve learned what debt we have on her. She has left us a legacy of practical knowledge to enable the survival and continued development of nursing.

The nurse patient relationship has 3 phases; the Orientation phase, working phase and the Resolution Phase. Each phase find ways in expression and require intervention.

I`ve also found out that in order for us to attain a healthy life we must have stress management and we must not stress others because this is not only applicable in clinics and hospitals but also in our daily living.

As I read the journal, for me, there’s one thing that the author put his emphasis about Hildegard Peplu’s ideas in nursing,” this is that “a human cannot function solely instead one must have an interpersonal relationship with others in order the accomplishment be met”, especially in the field of nursing, a nurse cannot function and cannot effectively deliver her nursing care without a relationship existing between her and her patients as well as the physicians, so as the patient without the nurse. From which through communication, the nurse and the patient’s relationship evolves and also therapeutic communication is a cure of disease.

Based on the journal, the closeness within the relationship of the nurse and the patient is necessary to build trust, empathy, enhances self-esteem and fosters growth towards healthy behavior. And the different phases that a nurse and the patient undergo which has a unique process which the nurse’s role changes from every phase, and with each role she must function effectively to attain the desired goals which is the wellness of the whole person.

I'm very thankful to Peplau because of the discovery of the "Psychodynamic Nursing" which has a big impact to the nursing profession today.

I think it's a very interesting article, because I fully understand the meaning of what is a nurse patient relationship and what it's effect on the patients who need this, and this Psychodynamic theory teaches me that smile is really contagious, because when I apply it to me former classmates in highschool and smile and say hi! to them, they answer me back with a smile too. I find it very intriguing issue about a person's behavior.
Also after some discussions of this theory and the autobiography of the one of the greatest theorist of all time,Hildegard Peplau, I learned that every people must have to manage their stress in order for them to be healthy and it is also an essential for me as an inspiring nurse someday.

In dealing with our patient, we should know how to handle their emotions and control their anger. We, the nurse, should not allow the patient to stress themselves, and stress other people too.


Besides from the knowldge I fetch from the journal reading, I also learned from my experience as a peer counselor that we should know how to talk and how to handle situations with our counselee, or the patients and respect their privacy. It only proves that Peplau's theory is not only for nursing practice but it is also applicable to our daily living.

I was really amazed and surprised upon hearing her life story. Her life story was incredible and really different to other nursing theorist. Her theory is very useful to people and to nurses. It helps to improve the quality of relationship of nurses to their patients especially to those people that are not given special care and attention by people. As i read this article, I found out that I can control my stress and can avoid making stress by properly managing my stress. Anxiety is contagious. Let me share my experience. When I was a fourth year student, I was elected as the SBO treasurer. I always have so many things to do in fulfilling the needed requirements during clearance days and also as a treasurer, I am obliged to collect fines of the late comers. I easily get irritated because, when I am doing something, late comers approached me, that's why sometimes, then, I get mad to others who never do bad something to me. now, I can handle my stress.

Hildegard Peplau deserves the title “mother of psychiatric nursing”. She gives a big contribution to the nursing practice and makes the nurses professional. She shows that the nurses need to have knowledge, skills and most of all attitudes to relate the patient to get their health back to normal. Her concept phases of nurse-patient relationship, helps the nurse and patient to make the treatment easy. She introduced other role of the nurse not only nurse as a helper, like nurse and patient are stranger to each other, should be a resource person, can be a teacher, leader, or councilor and of course nurse is a temporary care giver where in after the successful treatment the nurse and patient end their relationship.

This journal reading about the life and work of Hildegard Peplau is so precise and easy to understand, there are lots of information written here that is not written in the book, she is working with passion and it shows in all her assumptions. Because of her i learned that anxiety is contagious and smiling will be a great help to make bond tighter with others. All her contributions are very significant to nursing field and it is still used until now not only by nurses but also b most of the professions.

peplau was one of the greatest contribution in nursing profession. she was one of the nursing theorist that i admire next to nightingale. her theory focusses on psychological aspect of an individual which has a great influence in a recuprating patient--if he has so much problem, ofcourse his health wud decline. sometimes, one of the things that the patient lacks is acceptance--it's hard for hi to accept his situation i.e., the patient is supposed to be amputated because of an accident, its hard for him to think that one part of his body is missing and because of this, he will get angry to the world. in this case, the nurse will do the counseling in able for the patient to understand why they should do that certain operation. the nurse will tell the patient that he is still lucky, it's just a part of the body that has been lost in the accident not his life, thus the patient will be enlightened.

Peplau's psychodynamic nursing has really helped the nurse treat the patient's emotional and spiritual condition by manipulating her attitudes towards her patient. I remembered my sister, also a student nurse, telling me a story of how she mistreated the patient because the patient would not want to take her medication. My sister, out of her temper, walked out the door of the room and left the patient untreated with the medication. I know it wasn't ethical to treat a patient with an attitude. Peplau taught us to care for the patient meaning we have to understand their irritability to the presence of nurses instead of them having to understand us nurses. We, nurses, should change and follow the necessary guidelines of Peplau to improve our Interpersonal Relations with our patients.

This journal of Hildegard Peplau, the Psychdynamic Nursing helps the nurse to identify the importance of the relationship of the patient and the nurse.That the patient should expresses needs which find expression and require intervention in unique ways.
She described four phases of this relationship: orientation, in which the person and the nurse mutually identify the person's problem; identification, in which the person identifies with the nurse, thereby accepting help; exploitation, in which the person makes use of the nurse's help; and resolution, in which the person accepts new goals and frees herself or himself from the relationship.
Her idea in nursing, is an educative instrument, a maturing force, that aims to promote forward movement of personality in the direction of creative, constructive, productive, personal and community living.

Upon reading the journal, I came up with the idea that Psychodynamic Nursing is of great help in the nursing practice. This way, patients will get to participate and cooperate with nurse. And this is through the formation/development of the good relationship between the two.

With what Hildegard Peplau's lifelong journey in nursing field, she gave 50 years of her life for the benefit of nurses and its profession. Even if her study is already obvious in our daily routine, her Psychodynamic Nursing theory exhibit more of what is beyond the common sense can explain. This also equip nurses in facing various situations through assessing what role one should perform and at what stage you should start to provide quality care. Indeed nursing is a delicate profession because not only skills and attitude is present but especially knowledge and passion for you to succeed in the profession.

This article made me realize that I should do more with this profession than just being a traditional nurse. All that Peplau contributed had made me to strive more to serve and care for other people. Why? Because now I know those significant roles of a nurse that was taught by Peplau. It is my personal concern to apply it because I was given the chance to know it.

This theory of Peplau is very important for mentally illed patients and to all. It is our debt to Peplau that she really deserves to be honred by people of today.:)

Hildegard Peplau defines psychodynamic nursing because her model evolves through this type of nursing.She also described the nurse-patient relationship as a four-phase phenomenon.In Psychiatric Nursing or the nursing of the mentally ill,the concepts introduced by Peplau were so important in the practice of this discipline that nursing leaders formulated a therapy out of it.

For me,the Four Phases of nurse-patient relationship is very helpful in nursing students in approaching his clients.These are:orientation,identification,exploitation,and resolution.Her theory proposed the Interpersonal Therapeutic Process as I what I've read,her journal is particularly useful in helping psychiatric patients become receptive for therapy.

Otherwise,Peplau describes six nursing roles that emerge in the various phases of the nurse-patient relationship:(1)role of the stranger,(2)role of the resource person,(3)teaching role,(4)leadership role,(5)surrogate role, and (6)counseling role.

Peplau is really a great theorist,even though she died at the age of 89,she left a remarkable theory on the profession and on the lives of the mentally ill around the world which is very helpful
