MAJOR CONCEPTS AND DEFINITIONS

Psychodynamic nursing describe the dynamic relationship between a nurse and a patient.

Psychodynamic nursing is being able to understand one’s own behavior to help others identify felt difficulties, and to apply principles of human relations to the problems that arise at all levels of experience.

Peplau challenged psychiatric nursing to thrive in the new millennium in four central areas:

- the nurse-patient relationship

- engagement in evidence-based practice

- competence in information technology

- leadership in shifting the health care paradigm to community-based delivery.



NURSE-PATIENT RELATIONSHIP

(Interpersonal Relations Theory)

She described four phases of this relationship:

  1. Orientation - in which the person and the nurse mutually identify the person's problem
  2. Identification - in which the person identifies with the nurse, thereby accepting help
  3. Exploitation - in which the person makes use of the nurse's help
  4. Resolution - in which the person accepts new goals and frees herself or himself from the relationship.


Orientation Phase

1. When the nurse and patient first meet is known as the orientation phase.

2. This is a time when the patient and nurse come to know each other as people and each other’s expectations and roles are understood.

3. The patient at this time needs to recognize and understand their difficulty and the need for help, be assisted to plan to use the professional services offered, and harness the energy derived from felt needs.

4. It may be expected that the patient will test limits in order to establish the integrity of the nurse.

5. The tasks of this phase are to build trust, rapport, establish a therapeutic environment, assess the patient’s strengths and weakness and establish a mode of communication acceptable to both patient and nurse.

6. When the patient can begin to identify problems the relationship progresses to the working phase.


Identification Phase

1. Trust begins to develop and the patient begins to respond selectively to persons who seem to offer help.

2. The patient begins to identify with the nurse and identify problems, which can be worked on.

3. The meaning behind feelings and behavior of the nurse and patient are explored. Peplau states that when a nurse permits patients to express what they feel, and still get all of the nursing that is needed, then patients can undergo illness as an experience that reorients feelings and strengthens positive forces in the personality.

4. The tasks of this phase are to develop clarity about the patient's preconceptions and expectations of nurses and nursing, develop acceptance of each other, explore feelings, identify problems and respond to people who can offer help.

5. In particular the nurse assists in the expression of needs and feelings, assists during stress, shows acceptance and provides information.

6. The nurse and patient may make plans for the future but the implementation of the plan signifies the beginning of the exploitation phase of the working relationship.


Exploitation Phase

1. The patient realistically exploits all of the services available to them on the basis of self interest and need.

2. The nurse assists the patient in their efforts to strike a balance between the needs for dependence and independence.

3. The plan of action is implemented and evaluated. The patient may display a change in manner of communicating, as new skills in interpersonal relationships and problem solving are developed.

4. The nurse continues to assess and assists in meeting new needs as they emerge.


Resolution Phase

1. The resolution phase involves the gradual freeing from identification with helping persons, and the generation and strengthening of ability to stand alone, eventually leading to the mutual termination of the relationship.

2. The patient abandons old needs and aspires to new goals.

3. She or he continues to apply new problem solving skills and maintains changes in style of communication and interaction.

4. Resolution includes planning for alternative sources of support, problem prevention, and the patient’s integration of the illness experience.


Psychological Mothering:

  1. The patient is accepted unconditionally to satisfy his needs.
  2. There is recognition of and in response to the patient’s readiness for growth and his initiative.
  3. Power in the relationships shifts to the patient as the patient is able to delay gratification and to invest in goal achievement.


NURSING ROLES

1. Stranger Role: Receives the client the same way one meets a stranger in other life situations; provides an accepting climate that builds trust. Accepting the patient objectively.

2. Resource Person Role: Answers questions, interprets clinical treatment data, gives information. Interpreting the medical plan to the patient.

3. Teaching Role: Gives instructions and provides training; involves analysis and synthesis of the learner's experience. Offering information and helping the patient to learn.

Peplau separates teaching into two categories : Instructional—which consists largely of giving information and is the form explained in educational literature. Experiential— which is using the experience of the learner as a basis from which learning products are developed.

4. Counseling Role: Helps client understand and integrate the meaning of current life circumstances; provides guidance and encouragement to make changes. Working with the patient on current problems.

5. Surrogate Role: Helps client clarify domains of dependence, interdependence, and independence and acts on client’s behalf as advocate. Figuratively standing in for a person in the patient's life.

6. Active Leadership Role: Helps client assume maximum responsibility for meeting treatment goals in a mutually satisfying way. Working with the patient democratically.



MAJOR ASSUMPTIONS

Peplau identifies two explicit assumptions :

1. The kind of person the nurse becomes makes a substantial difference in what each patient will learn as he receives nursing care.

2. Fostering personality development toward maturity is a function of nursing and nursing education. Nursing uses principles and methods that guide the process toward resolution of interpersonal problems.

Nursing

Nursing described as a significant, therapeutic, interpersonal process.

Person

Peplau defines person in terms of man. Man is an organism that lives in an unstable equilibrium.

Health

Peplau defines health as a word symbol that implies forward movement of personality and other ongoing human processes in the direction of creative, constructive, productive, personal, and community living.

Environment

Peplau implicitly defines the environment in terms of existing forces outside the organism and in the context of culture, from which mores, customs, and beliefs are acquired.

Psychodynamic Theory of Hildegard Peplau helps the nursing profession a lot.. Because of her theory it can help us a lot regarding her nurse-patient relationship.

This theory teach us how to get along with the patient.Just like what the story that u've tell us that in nurse-patient relationship we need to understand the patients situation and we need to have a smile in our face always.

Hildegard Peplau's Psychodynamic Nursing theory focuses on how the nurse should appraoch their patients in order to have a good interaction and for quick development in terms of thier wellness.In Peplau's theory,she gave emphasis on how to perform the different roles that a nurse should do.Basically,a nurse plays an important role to the patient's life because not only he/she gives care but also the nurse gave knowledge and learnings to the patients that the patients can use for the rest of thier life.

The psychodynamic nursing is beneficial for the nurses and nursing students. The theory tackles about how can a nurse understand a person. She also stated the four phases of her theory namely the orientation, identification, exploitation and resolution which is very true. With this 4 phases of her theory we can also apply the roles of the nurse to a patient.Knowing her theory would help us to be an effective nurse.

Peplau's concept of Psychodynamic Nursing was a great help in our chosen field. It teaches us how to communicate with our patient which is very helpful in both party. Relationship with the patient is the most important skill a nurse must possess. Through this, we will be able to apprehend their behavior to help them with their dilemmas. It is not only to have relationship with the individual but as well as to provide therapeutic treatment through communication.

Building a relationship according to Pepalau has four phases. From knowing each other first, developing trust within each other to exploitation and to being strong enough to stand in their own. It is necessary that the nurse and the patient are going along well for the success of the outcome of the interaction. In this, we can be an active instrument to help a patient overcome his/her illness and for the betterment of his/her health status.

Peplau’s Psychodynamic Nursing has its four interconnected phases which help nurses deal with patients. It begins with the orientation phase wherein the nurse and the patient adjust to each other and try to get along with each other’s presence. It is followed by the identification phase where the nurse reorients the patient and tries to let the patient accept her illness. Then, the exploitation phase follows where the patient gets or benefits form the nurse’s help and the resolution phase where there would be an evaluation whether goals are met or established in the patient.
Another interesting part of the theory is the different roles the nurse plays on the patient. All those roles are set up by the nurse to create a relationship between him and the patient to further nurture the relationship and the nursing process.
As a nurse, reading through Hildegard’s theory bit by bit, constructs an ego in me to care for my future patients and comprehend the emotions and feelings he will show me on the process of healing him.

Psychodynamic Nursing of Hildegard Peplau is very abundant in the field of nursing. Peplau defines the nurse-patient relationship in four-phases. First the ORIENTATION where the nuse must assists the patient in recognizing and understanding the "patient" experience. Second the IDENTIFICATION where the nurse and the patient explore the experience and the needs of the patient which leads to a feeling of relatedness. Then the EXPLOITATION in this phase the patient derives the full value of the relationship as he moves on from dependent role to an independent one. And the RESOLUTION the last phase where even the relationship of the nurse and the patient was ended , it is very apparent that the experience leaves a lasting impression on the patient since illness and assuming a dependent role a unique human experience. The nurse-patient relationship, the nurse plays different roles which empower and equip her in meeting the needs of the patient. These roles are being used by nurses in different situations. These roles are Role of the Stranger, the Resource Person, Teaching Role, Leadership, Surrogate and Counceling. Her theory was simple, general, and empirical precise.

Because of this theory, we were able to understand mentally illed patient. Before we used to laugh and make fun of them but when Hilda came, this culture has been stopped. Peplau's theory is very essential for nursing. In order to help a patient achieve wellness, a nurse must not only render physical care. He must work hand-in-hand with the patient.

Peplau made importance here on the different roles of a nurse. Roles that will help the patient recover not only physically but also mentally and emotionally. Also the relationship of the nurse to the patient, physician and to the healthcare team. A nurse works dependently following the prescribed plan of care. And a nurse works interdependently with the team. You must know how to work with a group as told to us by Mrs. Oloresisimo.:)

This theory is being widely used today. And as a nursing student, I will apply this.:) It will enable me to establish a good relationship. It will be a great help for me to enjoy this profession.:)

Hildegard Elizabeth Peplau..the one who made the psychodynamic theory. This article was really good. It helps me understand how to interact to the patients in proper way, especially to the ill patients. Her theory was really great, like i've said before, without her theory, we can never ever learn how to interact with the patient who are mentally ill. And we can also use this theory in our every day living. I must say that this theory is my favorite theory among other theories. It really inspire me a lot, and helps me in everything. Peplau's works were very impressing,she really helps the nurse to make their patients trust her.Her four phases helps on how the nurse influence her patient and how to interract with people of different personalities.
She helps the nurse to make their patients accept their goals.
As it says that first impressions last,so it is really hard to do the orientation phase,when you fail it, you surely can't proceed to the next stage.

Her theory helps the nurses to interact with their patients and understand their situation. The interaction between the nurse and the recipient of care can really contribute in stress management, and also by this verbal communication, the nurse can easily share his/her life with the patient.

Psychodynamic Theory of Hildegard E. Peplau’s theory, which is the Interpersonal Relations Theory, as I can see emphasized the nurse-patient relationship. A nurse and a patient must really have a good relationship in order for them to attain the goal which is the patient’s better and faster recovery. A nurse should know how to gain the trust of her patient. Then good interaction and communication between the nurse and the patient will spring next.

I, as a nursing student can greatly apply her theory in my coming duties on the next semesters and in my profession hopefully as a registered nurse in the future.

A nurse must be able to endure all the unnecessary involvements that he will experience during his contact with his patient. It is really amazing and wonderful knowing that someone, Hildegard Peplau, formulate a theory that is really helpful not only in nursing but also in our daily lives as well. Psychodynamic Theory of Hildegard Peplau is applicable in research,education and practice. The 4 phases was described by her show the process to how to deal or to interact with the patients. Nowadays,one of the medical problems is some poor patients not admit in other hospital because of lack of money.To solve this problem the health care team or medical team adapt the Psychological Mothering which is accepted the patient unconditionally to fulfill his needs.
For me,this theory is the most applicable among all theory.

After I've read Peplau's Psychodynamic Nursing I've realized the importance of establishing a good relationship towards the patient. Through her theory we must follow the Four Phases to know more about the patient. First is the Orientation Phase, in which the nurse and the patient recognize and understand the experiences of the latter. The second is the Identification Phase, in which they relay their expirience and the needs of the patient towards the feeling of relatedness. The third one is the Exploitation Phase, in which the patient develops the significance of the new found relationship and moves from the dependent to independent one. And the last is the Resolution Phase, in which the patient and the nurse formulates new goals. Being a psychiatric nurse helps a lot in dealing with the patients.

Hildegard Peplau's theory gives emphasize on the therapeutic nurse-patient relationship. She describe this relationship in the four (4) phase phenomenon namely the orientation, identifisation, exploitation and resolution. This four phases shows the stages of a nurse-patient relationship from admission of the patient until the time of discharge. She also cited the roles of the nurse which gives the nurse the idea in identifying which role is appropriate in a certain situation together with the nursing process. These roles are applicable in different settings.

I like the persistency of Peplau when she chose work between marriage because during her time , most of the women chose to get married instead of having a working career. I admire her for so many things she had achieved and contributed to nursing profession like having the title of "Psychiatric Nurse of the Century" and the Interpersonal Relations theory she had formulated. I also admire her for being the pioneer of psychiatric nursing and having numerous writings and reseach featured at the American Journal of Nursing and awards like the prestigious Christiane Reimann Prize at International Council o f nurses Quadrennial Congress and a fellowship at the American Academy of Nursing.

Hildegard Peplau's theory gives emphasize on the therapeutic nurse-patient relationship. She describe this relatioship in the four (4) phase phenomenon namely the orientation, identification, exploitation and the resolution. Thias four phases shows the stsges of a nurse-patient relationship from the time of admission until the time of discharge. She also cited the roles of the nurse which gives them the idea in identifying which role is appropriate in a certaian situation together with the nursing process. These roles are applicable in different settings.

It specialize the nurse patient relationship which developed social skills of a nurse in order to fasten the recovery of the patient. This theory is clear, easy to understand and properly explained. Peplau`s Interpersonal Relations Theory made way for the nursing leaders to formulate a therapy out of it. She described the nurse-patient relationship as a four-phase phenomenon. The Interpersonal Therapeutic Process is based on the theory proposed by Peplau and particularly useful in helping psychiatric patients become receptive for therary. In her theory, she stated six nursing roles which enable and furnish the nurse in meeting the needs of the patient in different nusing situations and phases of Interpersonal Relationship.

Her theory gave us an idea on dealing with the patients properly in order to attain our goal. .

My understanding on Peplau's theory which is the Psychodynamic Nursing is if the nurse is not on the mood,her patient will also feel the same way.And in contrast,if the nurse is happy,her patient will also be happy.Her theory simply means that the mood of the nurse will affect the patient.

After reading this article about Hildegard's psychodynamic nursing theory, I totally understand what she want us to understand that it is not only the external side that we must focus on to achieve the quality care that a patient must get from us but we should always be aware that what is inside is the best. Patients are not the same, they have different attitudes that can be inevitably encouter by nurses, and as a nurse we must fulfill whatever the patient and even you have lacked.
good relationship with patients must be develop by each of the nurses, because it really helps the patient to be comfortable, gain trust and feel safe with you.
There are certain phase for the nurse to attain this kind of relationship, the orientation,identification,exploitation and resolution phase. These are steps that a nurese should know. and of course, the different nursing roles.

Peplau's Interpersonal Relationship theory made me realized that we must not only focus on the patients physical state but also on their emotional being. She described nurse-patient relationship as a four phase first the Orientation which the interaction of the nurse and the patient is developed. The second is the Identification which they strengthen their relationship by sharing their experiences. The third is Exploitation which the patient becomes independent and the nurse project new goals. The last one is the Resolution which the patient can do things independently
and this is where the relationship of the nurse and the patient ends.

PSYCHODYNAMIC NURSING of Hildegard Peplau help the nursing profession to develop its practice, research and education..most of the nurses in this present generation should know her, because it is all about the interpersonal relationship between the nurses and the patients.I've really appreciate her theory because good communication is needed to strenghten relationship of the nurse and patient. We, as a nurse are expected to be not only a health provider but also a care provider with the therapeutic practice. Peplau's theory also teach us the stress management, on how to deal with people who have a lot of stress in life especially those who are sick.

The psychodynamic nursing helps the nurses and nursing students to treat every other patient the care they need. Through this theory the nurses knows how to communicate to their patient. This theory helps nurses to give the care to the patient in all the aspects of life. This theory helps the nurse and the patient to share their life with each other from the day the patient admit to the hospital until the day the patient discharged.

She conceptualized the four phases of relationship. The orientation, identification, exploitation and resolution. I like Peplau and her work very much. Without psychodynamic theory, there is no nurse-patient relationship. The nurses will spend a hard time in assessing their patients. Sometimes it may result in errors and complications to the patient.

Peplau's theory helped us how to handle and how to analyze different situations as we encounter different patient's personality. "We can never please everybody." But through this theory, we can help our patients in accepting the fact that their need to undergo certain medications in the hospital and help them relieve their sickness easily.

For me, this theory will really guide nursing students to their clinical practice and I think without this theory nurses cannot take good care of their patients easily and has a big effect to the nursing profession today.

I think everyone should know Peplau’s works, so we would find no difficulty in handling patients and understanding their feelings and emotions, and also to be encourage, because Peplau is a courageous woman, courageous in such a way that she left us a legacy of practical knowledge which is there is no any good or better university can teach us, to enable us survive and to continue develop about nursing. Peplau is one of the theorists that can be considered as EXCELLENT because of her big contribution in the field of nursing and also because of her different experiences. She really deserves to be called as the “Mother of Psychiatric Nursing”. And I, as nursing student I will always remember and apply her contribution to help me in my chosen profession.......

Of all the theories we have studied, I believe that this theory, the Psychodynamic Nursing Theory, defined the essence of nursing, what really a nurse do and what really a nurse must be.

Nursing is equated to caring. A nurse, in order to render care must surpass the 4 phases of nurse-patient relationship, namely: orientation, identification, exploitation and resolution.
(well, i guess nursing should be equated with patience too.. =)

Also, a nurse is like an artist. well not like what we see on tv, but because she performs different roles in a patient life once the chapter of a patient's life in a hospital started.
Those are:

3. teacher
4. conselor
5. surrogate
6. (and an)active leader

IN Peplau's psychodynamic nursing, that the nurse must know the problem of her patient. and after you know her problem you must find a solution that helps your patient to more cooperative in caring to her self and if you already know the answer in her problem it will be more easy for your patient to care her sefl wihtout thinking any problem or thinking any person.

Virginia Henderson, being a glamorous theorist also inspired me and taught me a lot about nursing. It is a very great help for me (being a nurse aspirant) to know the 14 basic needs of human. At least from now until then I can apply it to my daily life and in rendering health care to others. Now, I learned the factors that should be focused to maintain wellness of a human being (even it is physical, social, mental, emotional and spiritual).
Also, through Henderson I learned to cope in such relationships that nurses encounter. Through her, I acquire the knowledge to handle a nurse-patient relationship, nurse-physician relationship and how to be a member of a health care team. Henderson taught me how to be a substitute, helper and a partner nurse to the patient. She taught us to work independently while promoting the treatment plan prescribed by the physician and how to contribute on carrying out the total program of care without doing the responsibility of the other health care members.
Moreover, Henderson's definition of nursing is also a very great help for me as nursing student. It gave me wider perception about the nursing profession. Also, the article excerpted from Kristen Swason's, "Nursing is Caring Doing for the Patient" taught us that nurse should act ultimately to preserve others wellness. It defined very well the meaning of nursing's "doing for" the patient and this include quickly bathing, dressing elderly patient, comforting other, anticipating patient's needs, protecting others from undo harm and ultimately preserving the dignity of the one done for...
Truly, Henderson is one of the aspiring theorists that may guide the nursing professionals now and in the near future.

The psychodynamic theory of Ms. Hildegard Peplau, really give a big help in the nursing curriculum of today's century. It can necessarily applied in nursing practice, in nursing education and in nursing research. She implemented the uses of the four phase which we considered as a guide in applying this kind of theory in the patients health condition.
The different nursing roles really help us in our daily living. as enumerated it was the role of the stranger, role of the resource person, teaching, leadership, surrogate and counseling.
It has a limited world for application, it cannot be applied in the newborn baby and in the comatose patient.

The Interpersonal Relations Theory has four phases namely

1)Orientation
2)Identification
3)Exploitation
4)Resulution

This is the process of the phases by which nurses connects to thier patients,this interaction may lead to acquiring the patients trust on the nurse.It focuses on the interpersonal relationship, Psychodynamic nursing is truly a remarkable piece of work.

Peplau describes nursing as a significant, therapeutic, interpersonal process. And she defines person as a man who is an organism that lives in an unstable equilibrium. Her definition of this two factors of the metaparadigm in nursing describes her theory in all, she introduced four phases in the relationship on nurse and patient which is realistic, she also proposes different roles of nurses in the nurse-patient relationship, which are stranger, councilor, teacher, resource person, surrogate role and active leader to meet the needs of the patient and to help the patient become independent.

this psychodynamic nursing gives an important role in nursig practice.As a nurse we need to have a nurse-patient relationship to relates the patient experiences and to have a good interaction among the two of us.if i will become a nurse in my future i will always use this theory.

4 me, peplau has the unique contribution among the existing nursing theories. she gave more emphasis on person's psychology about a certain thing. by applying her theory by the nurse, she wud b a gud conversationalist not just to the patient but to the people around her.her theory has the widiest scope because it can be also applied in many professions such as marketing and advertising.it is also applied in travelbee's theory, human2human relationship.

Peplau is an achiever in the field of nursing and she also the mother of psychiatric nursing... on the other hand, nurses should be a counselor, teacher, stranger, leader or manager, surrogate, and resourcer of her patients....
And the 4 phases that nurses have:

Psychodynamic Nursing of Hildegard Peplau helps a lot in the nursing profession. Because of her theory it can help us a lot regarding her nurse-patient relationship.

Doing an evalaution on different stages of the patient-nurse relationship helps the nurses progress their nursing care plan. Also, it is essential for a nurse to know his/her purpose and role on the patient's life. These important things in nursing profession are pointed out and explained by Psychodynamic Nursing of Hildegard Peplau. Psychodynamic Nursing of Hildegard Peplau greatly contribute to nursing pratice, education, and research.I've really appreciate her theory because she not only emphasize the skills a nurse must possess but also the attitude toward the patient.

Another thing is the stress management,which is encounter by most of us. That "smile" will do a great help in nurse-patient relationship.

The focus on this relationship allows for the nurse and the patient to work together as partners in problem solving. The model encourages and supports empowerment of the patient by encouraging the patient to accept responsibility for the direction of his well being. The model while focusing on getting sick people well is also interested in health promotion and wellness, and not just treatment of the ill.

Hildegard Peplau contribute the Psychodynamic Nursing that help us to understand the behavior of the person... She emphasized the development of interpersonal relationship between the patient and the nurse....It was very significant in nursing profession because being a good communicator with the patient will surely take big part in the development of the patient's condition......
