Journal Reading (Peplau and the Psychodynamic Nursing)

Remembering Hildegard Peplau
By Richard Lakeman

On the 17th of March 1999, HildegardPeplau died at the age of 89, ending a nursing career, which spanned over fifty years. Peplau is often recognised as the ‘mother of psychiatric nursing but her ideas have influenced all fields of nursing. In 1948 Peplau completed her seminal work, ‘Interpersonal Relations in Nursing’. However, it wasn’t published until 1952 because at the time it was too revolutionary for a nurse to publish a book without a medical practitioner as a co-author. This work which has been published in nine languages, has perhaps done more to facilitate the development of nursing from an occupation to a profession than any other. Sills (1998, p.171) described Peplau as “…a woman of uncommon intellect, socialized outside the traditional 1940s model of nursing in the United States, who developed a paradigm of professionalism…. that has permeated every aspect of her long and distinguished career.”

Even if a nurse has never read any of the hundreds of articles or books, which cite and develop Peplau’s ideas, it is likely that she or he, will still have been influenced by them. Peplau suggested that nurses can and do make a difference to people in the context of interpersonal relationships, which have a discernible pattern, predictable phases and during which the nurse assumes roles such as counsellor, leader or teacher.

Some may recognise many of her ideas as being similar to those developed in role theory, cognitive behavioural therapy, symbolic interactionism, or reflecting ‘common sense’. A number of studies suggest that Peplau's theory remains a common frame of reference for most psychiatric nurses (Hirschmann, 1989). Some selected ideas from the works of Peplau (1952, p16) described nursing as "a significant, therapeutic, interpersonal process. It functions co-operatively with other human processes that make health possible for individuals in communities .... Nursing is an educative instrument, a maturing force, that aims to promote forward movement of personality in the direction of creative, constructive, productive, personal and community living". Lack of growth, for whatever reason, implies impaired health in the individual and basic human needs must be met if a healthy state is to be achieved and maintained (Stuart & Sundeen, 1987, p 46).

A number of assumptions are implicit in Peplau's original and latter writings (Forchuk, 1991). For example, the relationship of nurse and patient is influential in the outcome for the patient; People may assume a number of roles and have the capacity for empathy in relationships (Torres, 1986, p 171); People tend to behave in ways which have worked in the past when faced with a crisis (Forchuk, 1991); Anxiety and tension arise from unmet or conflicting needs, and the energy which arises may be harnessed into positive means for defining, understanding and meeting the problem at hand.

The nurse patient relationship is characterised by a number of overlapping phases with a number of therapeutic tasks or goals to be accomplished. During each phase the patient expresses needs which find expression and require intervention in unique ways.

Orientation Phase

The phase of the relationship, when the nurse and patient first meet is known as the orientation phase. This is a time when the patient and nurse come to know each other as people and each other’s expectations and roles are understood. The patient at this time needs to recognise and understand their difficulty and the need for help, be assisted to plan to use the professional services offered, and harness the energy derived from felt needs (Peplau, 1952, p 19). It may be expected that the patient will test limits in order to establish the integrity of the nurse. The tasks of this phase are to build trust, rapport, establish a therapeutic environment, assess the patients strengths and weakness and establish a mode of communication acceptable to both patient and nurse (Shives, 1994, p 91). When the patient can begin to identify problems the relationship progresses to the working phase.

Working Phase

The working phase incorporates the identification and exploitation sub-phases and the relationship may fluctuate back and forth as new problems are identified. During the identification phase trust begins to develop and the patient begins to respond selectively to persons who seem to offer help. The patient begins to identify with the nurse and identify problems, which can be worked on. The meaning behind feelings and behaviour of the nurse and patient are explored. Peplau (1952, p31) states that when a nurse permits patients to express what they feel, and still get all of the nursing that is needed, then patients can undergo illness as an experience that reorients feelings and strengthens positive forces in the personality. The tasks of this phase are to develop clarity about the patient's preconceptions and expectations of nurses and nursing, develop acceptance of each other, explore feelings, identify problems and respond to people who can offer help. In particular the nurse assists in the expression of needs and feelings, assists during stress, shows acceptance and provides information. The nurse and patient may make plans for the future but the implementation of the plan signifies the beginning of the exploitation phase of the working relationship. During the exploitation phase the patient realistically exploits all of the services available to them on the basis of self interest and need (Peplau, 1952, p 37). The nurse assists the patient in their efforts to strike a balance between the needs for dependence and independence. The plan of action is implemented and evaluated. The patient may display a change in manner of communicating, as new skills in interpersonal relationships and problem solving are developed (Forchuk & Brown, 1989, p 32). The nurse continues to assess and assists in meeting new needs as they emerge.

Resolution Phase

The resolution phase involves the gradual freeing from identification with helping persons, and the generation and strengthening of ability to stand alone, eventually leading to the mutual termination of the relationship (Peplau, 1952, p 39). The patient abandons old needs and aspires to new goals. She or he continues to apply new problem solving skills and maintains changes in style of communication and interaction. Resolution includes planning for alternative sources of support, problem prevention, and the patient’s integration of the illness experience.

Roles of the Nurse

The nurse may assume different roles within the relationship. The first role assumed by both the nurse and patient is stranger. This role requires respect and positive interest on the part of the nurse. The nurse may function as a resource person, providing specific answers to questions usually formulated with relation to a larger problem. As teacher the nurse assists the patient as a learner to grow and learn from experience. As leader the nurse may assist the patient as follower in a democratically implemented nursing process (Stuart & Sundeen, 1987, p 46). The nurse may be cast into surrogate roles by patients based on their significant past relationships. Considerable importance is also assigned to the role of the nurse as counsellor which is viewed as helping the patient integrate the facts and feelings associated with an episode of illness into his or her total life experience. Nurses may assume many other roles but in the context of the interpersonal relationship all aim to assist the patient to meet the goals of therapy, need satisfaction and growth (Stuart & Sundeen, 1987, p45).

Communication

Peplau (1952) expanded on a number of concepts drawn from developmental, psychoanalytical and behavioural theory in order to assist the nurse in understanding behaviour. Peplau (1952, p 290) defined communication as an interpersonal process involving the selection of symbols or concepts that go some way towards developing a common understanding. Anxiety is present in all individuals to some degree and is cognitively triggered by real or imagined, internal or external threats to an individuals security (Forchuk, 1991). The ability to empathise with the patient, and self-understanding are necessary requisites of the nurse as therapist. Stuart and Sundeen (1987) describe Peplau's concepts as belonging to a school of thought called the interpersonal view of behavioural deviations which hold that behaviour evolves within the context of interpersonal relations. A common belief within this theory is that by experiencing a healthy relationship with the therapist, the patient can learn to have more satisfying interpersonal relationships in general. Closeness within the relationship is seen as necessary as it builds trust, empathy, enhances self-esteem and fosters growth towards healthy behaviour. The process of therapy involves re-educating the patient in more successful ways of relating.

Nursing’s debt to Peplau

This brief and selective summary cannot do justice to Peplau’s ideas, which have been developed, expanded and put into practice over the last fifty years. It is my hope that more people will continue to access Peplau’s original work, which will highlight that she was primarily a pragmatist rather than an academic theorist. It captures the essence of what nursing is and can become, lending it a timeless quality. Peplau developed knowledge with practical import on learning, anxiety, hallucinations, interpersonal concepts, individual, family and group therapy. She articulated a framework for partnership and recovery, which continue to have significance for contemporary practice.

Peplau developed and taught in the first graduate programme in psychiatric nursing in the United States, assisted in the development of the first graduate programme in Europe and was instrumental in developing the role of ‘nurse specialist’. She was an advisor to the World Health Organisation, a member of the International Council of Nurses, a fellow of the Academy of Sigma Theta Tau, and has been awarded some of nursing highest honours. Few students struggling to complete academic studies would be failed to be impressed by Peplau’s credentials, which include no less than seven doctoral degrees!

Peplau (1987) viewed psychiatric nursing as a viable, complimentary and necessary alternative to psychiatric-medical treatments. She encouraged nurses to be proud of the development of nursing and challenged us to remember that… Somewhere, somehow, at some time in the past, courageous nurses determined these skills, learned them, fought for the right to use them, refined them, and taught them to other nurses. All nurses have an obligation to remember that part of nursing’s past, and to keep their own skills in pace with new opportunities for nursing into the next century. (Peplau, 1989, p.32), Peplau was one such courageous woman who has left us a legacy of practical knowledge to enable the survival and continued development of nursing. We are forever in her debt.

ahm..good evening mrs. oloresisimo, through this article, i fully understand the life story of Hildegard Peplau and her Psychodynamic Theory. I have noticed that a step-by-step procedure in caring the patient is very important in a sense that it includes getting to know each other between the nurse and the patient. Also, I have learned that as a nurse, i should know how to manage stress, and never give it to your patient. And after the class, I have found out that her theory or stress management must be applied not only on the hospital but also in anywhere you are going to go.

Ma'am Dani, I am very thankful to you because you really help us fully understand the different nursing theories.
Good Day!

"Psychodynamic theory by Hildegard Peplau", another theory that helps us to analyze the true meaning of nursing....
This theory helps us to understand others people situation and to develop what we called emphaty..
I learned that at first the patient and the nurse are strangers, and step by step they develop relationship which became their basement in achieving their common goal..
Nurses should develop a deep understanding on the behavior of the patient like what Hildegard Peplau have done...
This concept also wants to express that patience matters a lot..

Good Morning Mrs. Oloresisimo!
I think it's a very interesting article, because I fully understand the meaning of what is a nurse patient relationship and what it's effect on the patients who need this, and this Psychodynamic theory teaches me that smile is really contagious, because when I apply it to me former classmates in highschool and smile and say hi! to them, they answer me back with a smile too. I find it very intriguing issue about a person's behavior.
Also after some discussions of this theory and the autobiography of the one of the greatest theorist of all time,Hildegard Peplau, I learned that every people must have to manage their stress in order for them to be healthy and it is also an essential for me as an inspiring nurse someday.

And I Thank You Mam also for this article because I learned many from it.I hope you will upload about the autobiography the other great theorists sooner or later in order for me to read it and have some advance studies about TFN class...

Good morning Mrs. Oloresisimo
Hidegard Peplau, base on my understanding is a very brilliant and inspiring theorist for she formulated a theory, that at first I thought that it was a very simple one because Psychodynamic nursing is just a patient-nurse relationship that a nurse communicate with a patient and that's it. But as you understand it fully,it is not just like that, her theory will help you control your emotions and stress and will help you learn how to get along with your patient, that is the most important one to know, your example ma'am about your friend, that because of hildegard peplau's theory your friend manage to get along with her patient. What like to do is to practice peplau's theory to control my temper because of different factors that have a strong impact on me like stdying and others..Thank you ma'am for making this topic clear to us and thank you ma'am for the "chismis" that you give to us that made our discussions very interesting...

Psychodynamic Nursing is a very interesting topic, because it doesn’t only teach you on how to become a good nurse but also a good person because Psychodynamic Nursing also focuses on how to deal with others because it emphasized on establishing a good relationship between nurses and patient through communication, you can never start learning the patients feeling and attitude without getting oriented because the problem will never be solved without getting passed to this orientation phase, and after that the rest of the phase will come up, and you will finished it through the resolution phase where you teaches your patient to become independent, and you can never finish this phases without having a strong relationship with the patient

I need to tell the patient in slowly talk and how his/her attention focus to me because if I emotion was ahead, my patient and I was not understanding. I am the nurse so I must have long and wide knowledge to understand the patient.

As I read this article, i found out that the Psycho-dynamic theory has a step-by-step procedure to make the best intervention of your nursing career. A nurse and the patient have to know each other in their first meeting. Then when they know each other in a long run, they must have to introduce themselves to one another and it is the nurse's job to take care of the patient's personal problem. And lastly, the nurse have to let the patient to stand alone and face the new problems that will come in the future.

I have applied this theory to my fellow friends and classmates when they need a friend to listen to their problems. As a friend, it's my duty to listen and give some helpful advice that can solve his problems.

I realized that being a nurse is not just like giving treatments and care to the patient but also, he must consider the feelings of the patient to make a successful recovery.

It needs a step by step process to make the communication successful. I also learned the other roles of nursing which is beneficial in nurse-patient relationship. Thanks to Hildegard Peplau for creating the Psychodynamic Nursing that is useful not only in clinical setting but in all aspects of our lives.

During our class discussion about theory of Peplau, I compare this to Nightingale`s concept.

Sympathy vs. Empathy, What is the difference between the two?

Hildegard Peplau and her 'Psychodynamic Nursing' introduce the concept of empathy which is sharing of
something you may have had similarly experienced. It may be a sad feeling or a bad experience and upon knowing this, you do something that will make that person feel better.it means there is an interaction.

Nightingale, because of her status, there is no interaction between her and the patient she was
focused on rendering care, the important thing to her is to improve the condition of the patient. Which i think is sympathy
because it is where you can understand what the person is going through but you feel slightly distant.

Thats all I can share,.
God Bless you Ma`am danie.

After reading this journal, several ideas suddenly fill my mind. It was that for a nurse to fulfill all his duty, different phases of interaction should be successfully accomplished.

"How can one give someone something he doesn't have?"

In relationship to Hildegard Pepalu's Psychodynamic theory, that we cannot give something we do not have. For example, how can we achieve a good relationship with our patients if we do not share our lives with them. How can we establish good communication if we don't know how to communicate well?

In dealing with our patient, we should know how to handle their emotions and control their anger. We, the nurse, should not allow the patient to stress themselves, and stress other people too.

Besides from the knowldge I fetch from the journal reading, I also learned from my experience as a peer counselor that we should know how to talk and how to handle situations with our counselee, or the patients and respect their privacy. It only proves that Peplau's theory is not only for nursing practice but it is also applicable to our daily living.

Ma'am Dani, thank you for providing us this blog. It really helped us a lot. Ma'am thank you again....

I admire her in her observation in formulating her theory. She used communication techniques as her main intervention to her patient’s problem. She also made some nursing roles that will help the nurses to easily communicate with the patient.

Maybe Hildegard Peplau’s experiences during her practice also help her in formulating her theory. I also believe to the statement that an individual has its own attitude because they have their own reason.

Hildegard Peplau's life story was interesting. She gave much contribution in improving our quality of life. Her interpersonal relationship theory is very useful in exploring and identifying the needs of the patient. She touched the lives of others by his work. I learned that the nurse-patient relationship is a big help because they establish a good relationship with the patient, in that way nurses can easily understand, give sounds, and emphatic advices about the health problem being experience by the patient.

Hildegard Peplau must have had foresight to set the very essential foundation of psychiatric nursing thru her work but because during that time it was too innovate and maybe revolutionary, her work was not given chance to be published and learned in the nursing realm of practice.

Peplau had perceived exactly that a good relationship between a nurse and patient promote beneficial effect on patient and positive result in general. So that she explicitly detailed the must conditions necessary for nurse-patient interpersonal relationship.

I think is a very inspiring work done by Hildegard Peplau. Inspiring in the sense that as nurses, we can assure that the communication between us nurses, the patients, the physicians as well as the patient's relatives and friends, would be brought to better use in the promotion of better healthcare. I think that if everyone would know Hildegard Peplau's works we would find no difficulty in handling patients and even understanding their feelings and emotions, resulting to a bond between the patient and the carer.

Hildegard Peplau is considered to be the mother of psychiatric nursing.Yet for me Peplau is an admirable nurse too just like Florence Nightingale, which she provided nursing with a meaningful method. As I read her article I realize how important her theory in the nursing practice;the theory of Psychodynamic Nursing. This theory makes me understand more about psychiatry which stated the understanding one's own behavior to help others identify felt difficulties and to apply the practice to help them.I appreciate also her introduced therapeutic nurse-patient relationship which is very important towards the promotion of wellness and health of the latter. At first in her time,Peplau's ideas were not widely accepted but as time goes by,some researchers also used her model in their studies.

I really inspired in Hildegard's theory. In education, her theory helps a lot because it is being used as a manual of instruction to help us and graduate nurses alike in creating a significant nurse-patient relationship.

In my overall remarks or comments in Hildegard Peplau's work,I have nothing to say but it is so great and very inspiring also.In clear and general words,it has provided a really significant contribution to the nursing profession.

Once again,to our adviser,Mrs.Danela Oloresisimo,a very warm thank you for another great and memorable discussion about one of our brilliant theorist,Hildegard Peplau....

"Nursing is an educative instrument, a maturing force, that aims to promote forward movement of personality in the direction of creative, constructive, productive, personal and community living..."
That is one of the sentences that struck me while reading the article.Peplau described nursing as an interpersonal relationship. It said that there should be balance between the patient's indepence and dependence on the nurse.
As i continued reading, i can picture myself on duty in the hospital. Maam, as you said, not all will cooperate with the nurse and i am expecting that too after learning from your experiences. But as a Peplau follower, I will apply her theory. I can manage my patients' behavior by having a mutual interaction with them. Maybe at first, it will be hard for a beginner like me but i will learn...because nursing is a maturing force that promotes a productive living.

Maam thank you for teaching us the very basic of nursing...the theories.

Peplau was also an outstanding woman/nurse that everybody admire her. Through her, "the sick person should not cure only his body but also his mind," that was a great help to nursing profession. We should know the patient, what's on his/her mind, so that we can easily communicate with him/her and also to be a part of his/her life. Personally, this journal of Peplau gave me an idea and or a tip on how to deal with my patients in the near future. It heightens my understanding and knowing with the nurse interventions. Moreover, It made me realize that aside from being a nurse I have other several roles to comply with such as being a resource person, a teacher, a leader, a counselor, a mother, a change agent and a whole lot more. Its a tough responsibility for a starter to reach but I know Peplau's Psychodynamic Nursing can help us do the trick.

I hope that Peplau, in her Psychodynamic Nursing, will also emphasize the nurse to nurse relationship because for me its also a factor that a nurse have to focus on since I strongly believed that nurses need each other in the clinical practice.

when i first read the contents of my registration form for this semester, i got excited when i found out that i'll be dealing with Psychology as it was included as one of my subjects. but there's a question that flashed into my mind: what is its relevance in taking bachelor of science in nursing?

when i read hildegard peplau's biography and her contribution in the field of nursing, it's as if i found out the answer to my question...

psychodynamic nursing is intersonnected with psychology (well, it's kinda obvious as both of them bear the greek word: psycho...)and relating with peplau's work as a theorist,it became apparent to me that studying both subject matter is of great importance for a nursing student to easily comprehend and apply the underlying principles that they are offering.

dealing with anxiety and other stressors (which hinder the individual's state of well-being), which was suggested by her work, is such a very important matter that a nursing student must learn and bear in mind as she proceeds to the realization of her career.

this theory is vital not only to provide a better nursing care but as well as to establish a soun internal environment within the nurse' personality for her to be able to perform well...

Psychodynamic Nursing Theory of Hildegard Peplau marked our nursing field..

She was a women who acquires extraordinary intellect.

Her work have orientation phase which is the phase when the nurse and patient first meet. It is the time when the patient and nurse tend to know each other which is also known in layman's term as the "getting to know stage." The second one is the working phase, this phase incorporates the identification and exploitation. The third one is the resolution phase wherein the gradual freeing from identification with helping persons are invloved.

This were just a part of the work and all af it are worth it!!!

Her theory is significant for it will teach us how to deal with stress. It also made me understand that although a patient is really hard to intract with, we must control our temper and try to understand them. Her theory put a big emphasis on how to interact with others. It also points out that every person is different from one another, that everyone has its own strength and weaknesses and one must not compare a person to another person for they weren't the same and we as future nurses, must treat our each patients specially and do our best to understand them.

She is such an enormous person because of her contributions in nursing profession. In her theory, I learned how to deal with stress, anxiety or how to manage my emotions or behavior. Nurse and patient relationship must be well understood by every nursing student. For the reason that they could treat their patients easily and that, they can understand how they feel in regards to their sickness. A patient who is suffering from different condition needs the best approach for them to feel comfortable. Overall, for me, the reason or the main reason of Hildegard Peplau in her theory is to treat the patient in extraordinary way and to make them feel that even they were suffering from pain still they have the rights to treat as a human. In this case, every time a person got ill/sick they need to be approach properly by the nurse assigned.

Hildegard Peplau's works and concepts really inspires nurses and nursing students. Her theory, the "Psychodynamic Nursing", is very applicable in nursing profesion. It's main focus is about the true meaning of the relationship between the nurse and his/her patient, where a nurse should know how to give "emphaty" to the patient.

For me, Peplau wants us to know that nursing is not just a profession where nurses are just serving to their patient because that is their job..but nursing is a kind of commitment where nurses should know how to give respect, love, and care to their patient.

Good evening Ma'am,

Let's cut to the chase and make this simple...

Hildegard Peplau's theory tackles about the interaction of the patient to its nurse. It shows the nurse how to aid the patients in terms of physical, social, emotional, intellectual, and also spiritual well-being.

Furthermore, the theory explains and teaches the nurse about the phases of the relationship of the nurse to its patient which starts with orientation, identification, exploitation and finishes with resolution which is the time that their relationship ends. With these phases, we, nurses, can apply the theory as it suppose to be.

Based on my perception, this theory can improve not just health but also the emotional, mental, social, and spiritual well-being of the person with an assessment of the condition of the patient and a discussion to have an opportunity to gain accurate data of his/her condition.

To sum up the journal, I have evaluated that the concepts of this theory emphasizes about the relationship of the patient and nurse to improve health condition.

As I read this article I’ve found out that Peplau’s theory gives us the step-by-step procedure in caring the patient which includes the four phases of psychodynamic nursing. This four phases start from the moment of orientation up to the moment of discharge. This helps the nurse on how they can gain the trust of their patient. As Peplau’s insisted, the patient begins to respond selectively to persons who seem to offer help. Among the four phases, I can say that the most crucial phase is the resolution phase in which the nurse must able to help her patient to be independent. Within the relationship, nurse must able to do their respective roles on their patients. Thanks to Hildegard Peplau who formulated the ideas which influenced all fields in nursing.

Other than the four phase, I am also glad that Peplau introduced the different roles of nurses because it truly uplifts the importance of nurses in the society, for nurses doesn’t only functions as one but a 6-in-1tasks. I can also say that mutual relationship is a product of her theory. Psychodynamic Nursing not only helps the patient to give their trust to their nurse, but it also helps the nurse to increase her self-confident in the process of interaction.
Peplau’s theory, a way for the recovery of the patient, and I may say, it is also a means in the development of one’s characteristic to help build oneself to a better person.

In a relationship, it is very important that there is a trust between the two persons. The relationship will work effectively and will last forever. So Hildegard Peplau's Psychodynamic in Nursing is very important not only in nursing profession but also in our society nowadays.

In a nurse-patient relationship that first goal of a nurse is to get the trust of his patient in order to assess and cure him correctly. I am so amazed in the nurses who can handle irritating patients and get their trust as days passed by.If a become a nurse, I will always apply Peplau's theory because I know that it will help me very much in my choosen carrer. I will also do the thing that the most nurses forget to do, and that is giving advices and encouragements that a patient needs in order to gain self confidence.

Hildegard Peplau and her Psychodynamic in Nursing changed the quality of care in people and give a benifit to us.

Hildegard Peplau and her Psychodynamic Theory do a lot of changes in the field of nursing. Peplau started the nurse patient relationship. At first, the hospital disregarded that idea. But after disregarding the idea of Peplau, the health care team realized that it is really an important thing because without the interaction between the patient and the nurse, the recovery of the patient is hard to achive. And that theory of Peplau made me realize that the interaction between two or more person really exist. And the nurse-patient-relationship will help the patient to recover easily. In nurse patient relationship, there are four phases that must be follow. And those phases are a great help because nurse and patient not only gain trust from each other but also lead both person in entering each others life and be part of it.And i'm so thankful that there is no more discrimination of mentally ill people and that is because of Hildegard Peplau..because of her theory, the psychodynamic theory.

when this topic is discussed in our class i learned that as a nurse i need to follow the step-by-step procedure of Peplau or the Four phases of Nurse and patient interaction the orientation where the nurse and the patient introduce their selves to each other, identification in this phase the trust is being develop, the third phase is exploitation where the nurse assist the patient to balance between being independent and dependent and lastly the resolution phase where in this phase the relation of the nurse and the patient ends.

I learned how to manage my stress and that never give it to the patient.

Good afternoon Mrs. Oloresisimo.
After reading the article about Peplau's life, I've been amazed by her ideas and perceptions and mostly her dedication to Nursing not only as profession but also a vocation. The Psychodynamic Nursing is the most effective way of taking care of patients because of its step by step process that will lead to a good interrelationship between the nurse and the patient. I'm inspired with her theory because it involves trust and stress management that is very much important for a person specially for a nurse. We have discussed that too much stress may lead to mental disorder so we must thank Hildegard Peplau for her Psychodynamic Theory.

Ma'am, thank you so much for giving us the chance to learn more and I personally thank you for helping me become a real student.

Author Mr. Richard lakeman captures and put Hildegard's works into these pages...he carefully laid the right words to come up with this journal. And what really struck me are his last words,

"She encouraged nurses to be proud of the development of nursing and challenged us to remember that… Somewhere, somehow, at some time in the past, courageous nurses determined these skills, learned them, fought for the right to use them, refined them, and taught them to other nurses. All nurses have an obligation to remember that part of nursing’s past, and to keep their own skills in pace with new opportunities for nursing into the next century."

These words have left an impact to me...that as a student nurse, i hold in my hand an obligation to keep the passion alive and keep the fires burning...
because nurses are important persons. They have the ability to change the life of their patients through their encouraging words. They even have the power to reunite families...
Peplau,in her theory has shared us the secret behind these life-changing abilities of a nurse..
Great thanks to all the theorists who put all their efforts in coming up with the different nursing theories. ^^.

"nurses can do and make a difference to people in context of interpersonal relationship" this is the line that was retained in my memory when we tackled about hildegard paplau.

it was nurse's responsibility to treat and heal their patient's illness but through our simple ways of expressing our concern to them, it will really help in their fast recovery. a simple smile and by showing our care as provider will really motivate them.

peplau's theory was then regarded as a common sense theory but it really contributed a lot in the field of nursing most especially in the psychiatric nursing.

it also taught us the importance of nurse-patient relationship where our patients express their needs which look for expression and require intervention in unique and unusual ways.

this theory involved different phases such as orientation, identification, exploitation and resolution phase. nurses asume different roles such as teacher, stranger, resource person, leader and surrogate roles.
this theory will us help us in our future profession.

Actually, now I can say that as a nursing student, even though just a 1st year college, I have debt already over Ms. Hildegard Peplau because of her theory that I am now applying and cherishing, and hopefully in the future, I can use them as a nurse. This nurse-patient relationship is really important in every aspects. But actually, I couldn’t patch up that much. " I know I was born for nursing," this is what Hildegard Peplau believes, the "Mother of Psychiatric Nursing" and the "Psychiatric Nurse of the Century", who spend her whole life focusing on her nursing profession, and influenced the world and the people during her lifetime and even now.

With her "Psychodynamic Nursing", she aspired that nurses can and do make a difference to people in the context of interpersonal relationship.

I believe that God not just gifted us with a mind that can understand but with a heart that truly cares. Peplau with her empathy for others shows that she really cares for them especially the mentally ill person despite of their condition in life with their society.

Peplau is truly a "Living Legend", a person with an important role to play in our lives, and a person who occupy space for every individual. I can say that Hildegard Peplau is so devoted on her profession-nursing profession. Because she used all her talent and life on studying, she was so focused especially on the mentally ill person. She enjoys talking to the retarded persons to have study on them. She formulated her theory which is the Psychodynamic Nursing. This theory explains the benefits of the interrelation between the nurse and the patient in a sense that the nurse will be able to understand the deeper needs of his patients not only taking care of them physically...

In her theory, as a nurse we will notice the essence of communicating with our patients. It will give us a better result as we take care of our patients.

The Psychodynamic Nursing is easy to understand and it is also easy to apply.

As a nursing student, I realize that my chosen profession is not just like giving care to my patient but also understanding their feelings regarding their situation... Because of Peplau's theory, I understand that when you listen to your patients' emotional problems and act as a counselor to them, they will learn how to trust you... This will help me and also my patient to be more confident inspite of his suffering and illness...

This article helps me a lot in understanding the works of Hildegard Peplau.

She emphasized the nurse-client relationship as the foundation of nursing practice. At the time, her research and emphasis on the give-and-take of nurse-client relationships was seen by many as revolutionary. Peplau went on to form an interpersonal model emphasizing the need for a partnership between nurse and client as opposed to the client passively receiving treatment and the nurse passively acting out doctor's orders.

The essence of Peplau's theories is the creation of a shared experience. Nurses, she thought, could facilitate this through observation, description, formulation, interpretation, validation, and intervention.
For example, as the nurse listens to her client she develops a general impression of the client's situation. The nurse then validates her inferences by checking with the client for accuracy. The result may be experiential learning, improved coping strategies, and personal growth for both parties.

Unknown said...

Hildegard's theory inform us that as a care provider, we must first establish our relationship with our care recipient not just only to meet the desire knowledge with the patient's condition but to have a deeper relationship as a part of our field. We must also understand the feelings of the client to satisfy their needs and provide proper health care. We also know that her theory is applicable to our daily lifestyle.

Thank you and keep safe!,.hehe,.

Good Morning po, Mrs. Oloresisimo. The most important thing that comes to my mind after I read the story of Hildegard Peplau is the fact that communication plays an important role in the nurse-patient relationship as well as its interaction with the behavior and manners of the persons involved specifically, the patients.
Through this, I can say that if a nurse knows how to deal with his/her patients, knows how to motivate them and look positively in life, and knows how give hope to patients, it’s more likely that the patient can overcome his/her illness, or will die with at least a simple smile in his/her lips.
Nursing is not only about profession or a vehicle to move to States. Sometimes it’s better to look at it as a caring profession or a noble one. It’s all about how you carry your job well and putting your heart to what you are doing.

Hildegard Elizabeth Peplau left us a legacy of practical knowledge and a remarkable stamp in nursing profession specially to the mentally ill person aroud the world. For me, She is a very kind and affable person because in her time, she noticed and cared to the persons who lives in the world of hallicinations and ilussions. At her young age, she showed bravery for she had to choose to work for herslelf, rather than marrying someone. I am very amazed with her personality because she lovs her profession very much and she is very dedicated that for her, she will not marry someone else instead she will marry her profession.

Nurse patient relationship is important because it can influence the health, or how the patient will cope up in his disease or illness. It brings happiness to the heart of the nurse if her patient know her name.

She taght us not only to care or serve for the patient instead they must also know their nurses. And she taught us that we are serving other people not just because they are paying us but because we love our profession.

Through Hildegard Peplau, we had the term nursing profession instead of only nursing occupation. The nurses should make a difference in rendering care in every patient in each unique ways because each patient has their own unique qualities. And the interpersonal relationship between the nurse and the patient can help when the nurse assumes the different roles of a nurse. As a student, I must not forget the struggles of the first nurses who developed the nursing profession and who fought for their rights to apply developed care to patients. Hildegard Peplau- The mother of Psychiatric Nursing died on the 17th of March of 1999 at the age of 89. She completed her seminal work "Interpersonal relations in Nursing”. However it had been published until 1952. Because at the time it was too revolutionary for a nurse like her to publish a book without a medical practitioner as a co author. Her work was published in nine languages and was known by many. She showed that she is one of the great theories like the matriarch of nursing herself. Her beliefs about nursing showed many things. Her orientation phase showed that the nurse mutually identify the person’s problem. According to her it’s the start of the interpersonal relationship of the patient. They try to communicate to the workplace were the other working phase happens like exploitation and identification. Lastly after that her resolution phase end it all up. Roles of a nursing according to her was the different roles of nursing like stranger role, resource person, teacher role, counseling, surrogate role and active leadership role. For her communiction is also important for a healthy condition.
As a nurse aspirant we should learn to be more like Hildegard she is a true nurse theorist.

Hildegard Peplau introduced to us the nurse-patient relationship regarding with her theory psychodynamic nursing. I’ve learned that it is important that the nurse should help the patient on how to deal with their condition to be able to interact with them correctly and to have a good relationship with each other. A nurse must be responsible, humble, caring and knows how to cooperate with their patient especially with their different attitudes. I also learned that a nurse should know how the patient should listen to them in order to achieve the health properly. The four phases she made helped us to control and understand our temper in relations with the patient. A person was a unique individual that has its own strengths and weaknesses which a nurse should focus and understand. Her life enlightens me to become more useful, active and efficient nurse someday. Her theory help us also to become a healthy individual in mental, emotional, physical and spiritual well being to improve and to have a complete and healthy condition.

Peplau’s contribution to nursing profession is great, let us try to imagine that nurses are not educated to established rapport with their clients. I myself do not like the idea because I hate nurses who are moody, hard to talk with and especially those who do not smile at all, as if they are angry to the patient. Though Peplau already has this theory I still encounter nurses who are not applying it during nurse-patient interaction. I wonder if they have studied it, or perhaps their clinical instructor does not give much emphasis on it.
Thank you Ma’am for putting on our hearts and minds the theory of Peplau through your humorous example, “the best colostomy”. I will never forget every story you shared and those that are to be told. A very promising person who had contributed much in the nursing profession and still being able to influence many people in the said profession, Hildegard Peplau really had bestowed something in the field. Though her theory, for some, stands as a “common frame” of reference for most of the psychiatric nurses, some are still not convinced and doesn’t recognize her work as for them; ideas of hers are similar to those who developed role theory etc. But still many believed that her definition of terms in nursing is a common ground for them to be able to understand psychiatric nursing.

Recognized as the “mother of psychiatric nursing” only proves her theory and she as well is truly credible for the title.

Having read this article made me understand the importance of good communication skills for nurse.Good communication skills would help a nurse deal with his/her patient appropriately. Communication, I should say, is the most basic foundation in a nurse-patient relationship. With this, a nurse can earn the patient’s trust easily and can make them be involved in the care for itself. Possessing such good skill/ability would make a nurse properly identify the problem of the patient at hand, because the patient would personally say what they are feeling.

Hildegard Peplau made such a breakthrough in the field of nursing by developing a theory that would help every nurse to become an effective care provider. Nurses do not just deal with clinical aspects of a patient but also emotional aspects. With these, nurses should learn to understand how to deal with patients when it comes to their behaviors. Peplau introduced clearly the different roles of a nurse in a nurse-patient interaction. Nurses really play enormous roles depending on the patient’s needs. Peplau also introduced the phases or the steps in nurse-patient interaction.

Peplau is a very inspiring nurse to be studied because of her wonderful works or contributions in the field of nursing. Because of her works as what the author of the article had mentioned, it helped in making nursing a profession rather than just a mere occupation.

with this article, I can say I have just acquired useful information that I can use in the future when dealing with my future patients. it was very educational.

We should admire everything about Hildegard Peplau. On how she gives importance to the nursing possession and on how she elaborate every details about psychiatric. She developed different phases that help the students to undergo on that phases to achieve their goals in caring a patient. I can also say that Psychodynamic nursing of Peplau is very inspiring. Especially to people like us, as a nursing students. It was inspiring in the sense that we as a nurses we should assure that we have a good relation or communication with our patient’s, to the physicians that we are helping on and also to the family of the patient that we are caring of. In this aspect the “Interpersonal relations in nursing” of Peplau enters, meaning “the communication of one person to another”, through this practice it will lead us in the promotion of better health care. I think everyone should know Peplau’s works, so we would find no difficulty in handling patients and understanding their feelings and emotions, and also to be encourage, because Peplau is a courageous woman, courageous in such a way that she left us a legacy of practical knowledge which is there is no any good or better university can teach us, to enable us survive and to continue develop about nursing. Peplau is one of the theorists that can be considered as EXCELLENT because of her big contribution in the field of nursing and also because of her different experiences. First, the determinatlion of peplau to let nursing recognize as a profession not just an occupation. Although she is known is other field, she is more on the development of psychiatric nursing.

Second, She believe that nurses can make difference and changes to the people because of having interpersonal relationship. By means of that she give attention to those patient who need to be approach by nurses and nursing care of it.

And third is that, she give emphasize to have boundaries between the nurse-patient relationship and it is included in her phases of nurse-patient. She cited all the needs of the patient and achieve the aspects of a person (mental, physical, social, spiritual. etc.) in each phase. From the orientation phase upto resolution phase.

I can say about those things, she had a great concern with the interaction with each other and that her idea is since and now applied.

I salute Hildegard Peplau for her theory and the numerous awards that she has achieved.

HER THEORY..
she stated six nursing roles which are : the stranger role, resource role, teaching role, counseling role, surrogate role, and active leadership. *The stranger role is when client receives same way when one meets a stranger in other life situation. This provides acceptance which leads to trust. *The resource role gives information and answers questions. *The teaching role is when you give instructions. *Counseling is when you help them understand current life circumstances. *The surrogate role Helps client clarify domains of dependence, interdependence, and independence and acts on clients behalf as advocate. And lastly, *The Active leadership wherein it helps client assume maximum responsibility for meeting treatment goals in a mutually satisfying way.

She stated Developmental Stages of Nurse-Client Relationship which are : Orientation phase, Identification, Exploitation, and Resolution phases. These are very helpful in having a good and effective relationship with the patient. It helps more in their communication and interaction.
