Peplau's Psychodynamic Nursing

Psychodynamic Nursing

Theoretical Sources

- borrowed from behavioral science—psychological model

Use of Empirical Evidence

Maslow – people are motivated to attain self-actualization

Miller – personality theory, adjustment mechanisms, psychotherapy and principles of social learning

Pavlov’s – stimulus-response model influenced the principles of Miller’s social learning

Sullivan – the pioneer of modern psychiatry and includes cultural and social determiners to Freud’s interpersonal relationship model

MAJOR CONCEPTS AND DEFINITIONS

Psychodynamic nursing describe the dynamic relationship between a nurse and a patient.

Psychodynamic nursing is being able to understand one’s own behavior to help others identify felt difficulties, and to apply principles of human relations to the problems that arise at all levels of experience.

Peplau challenged psychiatric nursing to thrive in the new millennium in four central areas:

- the nurse-patient relationship

- engagement in evidence-based practice

- competence in information technology

- leadership in shifting the health care paradigm to community-based delivery.

NURSE-PATIENT RELATIONSHIP

(Interpersonal Relations Theory)

She described four phases of this relationship:

  1. Orientation - in which the person and the nurse mutually identify the person's problem
  2. Identification - in which the person identifies with the nurse, thereby accepting help
  3. Exploitation - in which the person makes use of the nurse's help
  4. Resolution - in which the person accepts new goals and frees herself or himself from the relationship.

Orientation Phase

1. When the nurse and patient first meet is known as the orientation phase.

2. This is a time when the patient and nurse come to know each other as people and each other’s expectations and roles are understood.

3. The patient at this time needs to recognize and understand their difficulty and the need for help, be assisted to plan to use the professional services offered, and harness the energy derived from felt needs.

4. It may be expected that the patient will test limits in order to establish the integrity of the nurse.

5. The tasks of this phase are to build trust, rapport, establish a therapeutic environment, assess the patient’s strengths and weakness and establish a mode of communication acceptable to both patient and nurse.

6. When the patient can begin to identify problems the relationship progresses to the working phase.

Identification Phase

1. Trust begins to develop and the patient begins to respond selectively to persons who seem to offer help.

2. The patient begins to identify with the nurse and identify problems, which can be worked on.

3. The meaning behind feelings and behavior of the nurse and patient are explored. Peplau states that when a nurse permits patients to express what they feel, and still get all of the nursing that is needed, then patients can undergo illness as an experience that reorients feelings and strengthens positive forces in the personality.

4. The tasks of this phase are to develop clarity about the patient's preconceptions and expectations of nurses and nursing, develop acceptance of each other, explore feelings, identify problems and respond to people who can offer help.

5. In particular the nurse assists in the expression of needs and feelings, assists during stress, shows acceptance and provides information.

6. The nurse and patient may make plans for the future but the implementation of the plan signifies the beginning of the exploitation phase of the working relationship.

Exploitation Phase

1. The patient realistically exploits all of the services available to them on the basis of self interest and need.

2. The nurse assists the patient in their efforts to strike a balance between the needs for dependence and independence.

3. The plan of action is implemented and evaluated. The patient may display a change in manner of communicating, as new skills in interpersonal relationships and problem solving are developed.

4. The nurse continues to assess and assists in meeting new needs as they emerge.

Resolution Phase

1. The resolution phase involves the gradual freeing from identification with helping persons, and the generation and strengthening of ability to stand alone, eventually leading to the mutual termination of the relationship.

2. The patient abandons old needs and aspires to new goals.

3. She or he continues to apply new problem solving skills and maintains changes in style of communication and interaction.

4. Resolution includes planning for alternative sources of support, problem prevention, and the patient’s integration of the illness experience.

Psychological Mothering:

  1. The patient is accepted unconditionally to satisfy his needs.
  2. There is recognition of and in response to the patient’s readiness for growth and his initiative.
  3. Power in the relationships shifts to the patient as the patient is able to delay gratification and to invest in goal achievement.

NURSING ROLES

1. Stranger Role: Receives the client the same way one meets a stranger in other life situations; provides an accepting climate that builds trust. Accepting the patient objectively.

2. Resource Person Role: Answers questions, interprets clinical treatment data, gives information. Interpreting the medical plan to the patient.

3. Teaching Role: Gives instructions and provides training; involves analysis and synthesis of the learner's experience. Offering information and helping the patient to learn.

Peplau separates teaching into two categories : Instructional—which consists largely of giving information and is the form explained in educational literature. Experiential— which is using the experience of the learner as a basis from which learning products are developed.

4. Counseling Role: Helps client understand and integrate the meaning of current life circumstances; provides guidance and encouragement to make changes. Working with the patient on current problems.

5. Surrogate Role: Helps client clarify domains of dependence, interdependence, and independence and acts on client’s behalf as advocate. Figuratively standing in for a person in the patient's life.

6. Active Leadership Role: Helps client assume maximum responsibility for meeting treatment goals in a mutually satisfying way. Working with the patient democratically.

MAJOR ASSUMPTIONS

Peplau identifies two explicit assumptions :

1. The kind of person the nurse becomes makes a substantial difference in what each patient will learn as he receives nursing care.

2. Fostering personality development toward maturity is a function of nursing and nursing education. Nursing uses principles and methods that guide the process toward resolution of interpersonal problems.

NURSING METAPARADIGM

Nursing

Nursing described as a significant, therapeutic, interpersonal process.

Person

Peplau defines person in terms of man. Man is an organism that lives in an unstable equilibrium.

Health

Peplau defines health as a word symbol that implies forward movement of personality and other ongoing human processes in the direction of creative, constructive, productive, personal, and community living.

Environment

Peplau implicitly defines the environment in terms of existing forces outside the organism and in the context of culture, from which mores, customs, and beliefs are acquired.

Theoretical Assertions

Interpersonal Relations in Nursing

  • Patient-nurse relationship
  • Awareness of feelings
  • Use of experiential learning method

Logical Form

- inductive approach

- empirical generalizations

- observes empirical events and generalizes from specific to all similar events

Application and Importance of the Theory

PRACTICE:

  1. Integration of scientific disciplines in formulating paradigm of psychiatric nursing.
  2. Strengthen nurse-patient relationship.
  3. The start of psychiatric nursing.
  4. Used for counseling women undergoing depression.

EDUCATION:

  1. Interpersonal Relations in Nursing—used as a manual of instruction to nursing students.
  2. Foundation of psychiatric nursing education.

RESEARCH:

  1. Different studies on the nursing phenomena.
  2. Improvement of the social system.
  3. Stress management program.
  4. Formation of behavior scale.
  5. Therapeutic behavior of the nurses.

Analysis of the Theory

SIMPLICITY

  1. The major theory that is interpersonal relations is easily understood.
  2. Assumptions and key concepts were clearly given, explained, broken down and outlined.
  3. Demonstrated clearly and identified properly the four phases of nurse-patient relationship and interpersonal model.
  4. Applied effectively in the nursing profession.
  5. It is consistent and semantic.

GENERALITY

  1. Applicable to all nurses in any setting.
  2. Used only in the situations that communication can occur.
  3. It is impossible in working with senile, comatose or newborn patients. (one-sided relationship)

EMPIRICAL PRECISION

  1. Based on reality.
  2. Could be tested and observed using pure observation.
  3. Theoretical area and empirical data are validated and verified.

DERIVABLE CONSEQUENCES

  1. Widens the perception of nursing profession.
  2. Broaden the scope of nursing practice.
  3. Touched the lives of many.

I believe that not only the knowledge and skills that acquired from school will lead us and the measure to be a sucessful nurse but it is how we applied the knowledge we learned. I've really appreciate her theory because good communication is needed to strenghten relationship of the nurse and patient. We, as a nurse are expected to be not only a health provider but also a care provider with the therapeutic practice. Peplau's theory also teach us the stress management, on how to deal with people who have a lot of stress in life especially those who are sick. As a nursing student of the current generation, we have a big gratitude towards these theorists that we are thankful that they contribute a lot in nursing profession.

Psychodynamic Nursing, the theory discovered by Hildeagard Peplau. Through this theory peplau focus on the nurse-patient relationship.

The psychodynamic nursing helps the nurses and nursing students to treat every other patient the care they need. Through this theory the nurses knows how to communicate to their patient. This theory helps nurses to give the care to the patient in all the aspects of life. This theory helps the nurse and the patient to share their life with each other from the day the patient admit to the hospital until the day the patient discharged.

For me, this theory will really guide nursing students to their clinical practice and I think without this theory nurses cannot take good care of their patients easily and has a big effect to the nursing profession today.

I believe that to become an effective nurse you should interact to your patient to give an appropriate cure to him. So Hildegard Peplau's Psychodynamic in Nursing which includes the nurse-patient interaction is very applicable in nursing practice nowadays. Like Environmental theory, Psychodynamic Nursing can easily understood because of its simplicity and being consistent. So the readers may easily understood the theory. I am truly amazed on her uncommon intellect. She conceptualized the four phases of relationship. The orientation, identification, exploitation and resolution. I like Peplau and her work very much. Without psychodynamic theory, there is no nurse-patient relationship. The nurses will spend a hard time in assessing their patients. Sometimes it may result in errors and complications to the patient.

While in research, the formation of behavior scale and the therapeutic behavior of nurses.

This theory focuses on the patient-nurse relationship, awareness of feelings and the use of experiential learning method. This have the four phases, the orientation, where in the nurse and the patient's first meeting. The identification, this is the time when the patient identifies with the nurse. The exploitation, the nurse exploits the all the services that is available. And the resolution, which is the patient abandons all needs and aspire to new goals.

during the Prelim examination in TFN, i got several errors in the application of Nursing theorists because i had this confusion between Peplau and travelbee.
as i read this journal, i could say that certain things regarding my confusions were finally sttled out.

"ate hilda" has a vast extension of her self in the field of rendering nursing care. she is indeed the mother and pioneer of Psychiatric Nursing which, when we come to keenly think and give focus on it, will give us a general thought:

peplau's psychodynamic Nursing theory is one of the foundations for a nursing student to reach her goal towards the continuity and reality of her intervention.

after the enrollment, i couldn't control my urge to read the tomey-alligood book and so i had reached up to abdellah's pages. my understanding regarding peplau's theory is considerably a very minute in the nursing sense because i thought upon reading that this theory is only intended for the mentally-incapacitated ones. then, the octaviano-balita TFN book was given and again, i read and finished reading it. somehow, certain things on my perspectives have changed.

with this journal (and of course, with the TFN discussions) it is clearly-cited and emphasized that peplau, as a nursing theorist has managed to sum up and then expand in her own interventions the thoughts of other persons to be able to come up with this theory. really, this one is based on reality and i, myself, to my greatest surprise has been practicing this theory ever since i wgained my friends and socilize with other people. the power of the contagious smile agsinst the communicable stress and anxiety.
peplau, is not only the mother of this nursing thepry theory, because i think, she's also one of those who give birth to the upcoming generations of individuals who will render qualitative nursing care.

good morning ma'am,

When we tackle this theory, The Psychodynamic Nursing or The Interpersonal Relations in Nursing by Hildegard Peplau, I realized and learned that a nurse must be connected to his patient because this nurse-patient relation is a great factor in fast recovery of the patient. I find it easy for me because I can get along with different people with different personalities. As a nurse, he must know the physical, emotional, spiritual, intellectual, and social being of a person not only the needs of the person. In my own point of view, a nurse must make his patient jolly. This is also one factor that affect the recovery of a patient. This theory is an important factor in the nursing profession to make you a successful nurse someday...

This article emphasizes the essence of interpersonal relationship between a nurse and a patient that has a great influence in the recovery of a patient. This Psychodynamic Nursing was one of my favorite theory because it is natural. Every nurse has a magic to help and cure the patient, and it is their own way of taking care of their patients, the nursing care. I learned that a nurse should always has a good mood and patience so that they can handle things in their own way.

The nurse should treat the patient right so the patient will feel comfortable and he will be able to be open with the nurse. he will not be afraid to tell the nurse what he is feeling.

Many of them may reason out that they what to serve man but as a whole for those student, they enrolled in this course because they want to earn dollars. Nurse-patient-interaction are very essential in this field because this is the only thing that nurse and patient can understand each other.

Unlike Nightingale's theory which focuses on the environment, because of her status, she lack one factor in attaining patient's recovery which is the nurse-patient relationship, while Peplau's theory focuses on this aspect.

For nurses to be successful, they should establish good relationship with their patient. And in establishing good realtionship, the nurse should pass the four phases of relationship according to the theory of Peplau.

Peplau's theory helped us how to handle and how to analyze different situations as we encounter different patient's personality. "We can never please everybody." But through this theory, we can help our patients in accepting the fact that their need to undergo certain medications in the hospital and help them relieve their sickness easily.

"Smiles are contagious", as coined by Ma'am Oloresisimo during our discussion, it really sticked on my mind that anger can really stress all the people around you, but we should not multiply this anger but instead, our smile should be given to each and everyone around us. We cannot give our patients the relieve that need in order for them not to feel the pain, so we should not give them the anger we had encounter later in the morning before we arrived on our duty but instead, we should provide them our beautiful smile so that the pain the feel will be relieved in less a period of time.

If Florence Nightingale’s theory emphasized on the environmental management that is important to patient’s recovery, Hildegard Peplau’s theory of nurse-patient relationship involves the stress management. This theory’s number one focus is to help the patient with his or her problem and help him or her to accept it in a good way and with a positive view of what will or what will be the effect of the solution.

We can pick out many benefits in this theory it include improving our social interaction, it practice the stress management and also we can apply it to anyone. This theory help me realize one of the important things that nurses should know to help her patient as well as his or her approach to the patient.

According to the journal and to my understanding,Peplau's Psychodynamic Theory was a very good theory. It's hard to communicate with a person who has a problem such as the insane ones. But Peplau could handle that kind of problem. The therapeutic relatioship was involve in her theory. And there are different phases that could help us (during our duty, etc). Those phases are Orientation Phase, Identification Phase, Exploitation Phase, and Resolution Phase. And base on what I have read and how I understand it, to have a very good relationship between the nurse and patient, first you must introduce your self to the patient and vice-versa. And in identification, trust was developed and the reletionship was starting to grow. In exploitation, manners of the patient was starting to change. And there are plans that was going to apply to have a best result of treatment. And in the resolution, the nurse lets the patient to become an independent. So when they separate from each other, there would be no difficulties. Having Peplau as a theorist in nursing field is a great honor because she's a very wonderful and intelligent person. She's really a "Psychiatric Nurse of the Century" like other people says.

Hildegard Peplau used the term, psychodynamic nursing, to describe the dynamic relationship between a nurse and a patient. Through this psychodynamic nursing, the nurses were able to share their life on their patient. Her theory was more on application rather than abstract. It is easy to understand and it gives us the idea that a nursing is not just like a profession but also a passion. This guides every nurse and the future nurses on how they will handle their patient from the very start up to the moment of their discharge.
Peplau described four phases of this relationship: orientation, in which the person and the nurse mutually identify the person's problem; identification, in which the person identifies with the nurse, thereby accepting help; exploitation, in which the person makes use of the nurse's help; and resolution, in which the person accepts new goals and frees herself or himself from the relationship.

Peplau's contribution in Nursing was a great help, because without her Psychodynamic Theory, nurses around the world cant help their patients in
their illness because their psychological connection to each other are not sufficient, making it difficult to the nurses to help their patients. But when Peplau's started to give ideas to the nursing practices, it helped a lot in giving care to the needy.

I really appreciate PHYCHODYNAMIC NURSING and its usefulness to nursing care because in this field we can communicate effectively with the patient or the interpersonal aspect of caring.

It is the interaction of the nurse to his patient.

Thanks Hildegard Peplau for contributing your knowledge and ideas that can add practice for our profession.

Hildegad Peplau's Pdychodynamic Theory for me is one of the greatest contribution a nursing theorist could give, people with mental illness are often taken forgranted in our community because it is hard to communicate with them and it seems hopeless to cure them.. giving her attention to the mentally illed people open the eyes of many other persons that these mentally illed people are not completely incapable, and communicating with them requires effort that are ordinary. Her "working with a smile" thought is also important, it reminds nurses and other people that smiling can be a major therapeutic act. And it is also important not to irritate other people bacause of out own problem.

Peplau's Psychodynamic Nursing whose main purpose is being able to understand one's own behavior to help others and apply the principles of human relations to the problems that arise was examined by Peplau after which the result of her work provided us much deeper and broader understanding of Psychiatric Nursing.

She introduced four phases in a nurse-patient relationship. These are orientation, identification, exploitation and resolution which really address therapeutic concern between a nurse and a patient.

Thus from these works, educators and nurse practioners nowadays all have clear focus on what to achieved as far as psychiatric nursing is concern.

Every one of us needs a person who will understand our problem or situation. In nursing practice Peplau’s Theory is very significant to everyone. In Interpersonal Relations in Nursing of Hildegard Peplau, I realized that nursing is not only a profession or a job but also devotion. Many students now choose nursing as a course for their stepping stone to go abroad and earn dollars, some of them didn’t perform their duty very well they only focus only on the high salary they get.

The therapeutic relationship is vital to nursing practice. The feeling of the patient will feel better if their needs are fulfilled or there is someone who can understand his predicament. We should analyze the feelings, emotions, and actions of the patient.

In Interpersonal Relations Theory, it helps us to cure not only physically but also mentally. Some patient doesn’t easily cure because of their mental problem. As a nurse we can help are patients by visiting them and support them that they can overcome what problem they have.

Psychodynamic nursing explains the relationship between the nurse and the patient. Proper caring will not workout if the patient didn't cooperate with the nurse.

The nurse must understand the behavior of the patient in order to know what's the patient's problem for her to help him. She stated that a nurse can be a stranger first, then a resource person, a teacher, a counselor and a leader. This theory will help in the develpoment of nursing care, a nurse can apply to her patient.

Hello Ma'am,
The main concern of Peplau's Psychodynamic Nursing is to improve and develop the relationship of a nurse and patient and the interaction between them as part of providing health care.

In my opinion I think Peplau's Psychonamic Nursing is the most well applied today in the Nusing profession because unlike Nightingale, she gave emphasize the factor which may help in the recovery of the patient, which is the well and nice interaction. Her unstoppable career also focuses on that prompting of nursing as a profession and for me it is very astonishing. In her work, she described the real essence of nursing by that the health care system should include the care of the nurses and its role as a nurse such as stranger, resource person, teacher, leader, surrogate and counsellor.

Honestly, before,I thought that once a nurse entered her patient's room, her only duty is to check his temp., his b.p., his dextrose. etc.But now, I learned that a nurse is more than that. She nurses, guides, and cares. One of her duties is to earn the trust of her patient and to earn it,she should learn those 'phases'. I think it will help alot!

Psychodynamic theory, focus more on the human behavior. In this theory, nurse-patient realtionship is important. Because for her it is important that the nurse knows his/her patient, and the patient must know his or her nurse. Communication plays a vital role in her theory. In the phases of her theory, the nurse should know or identify her client's problem. After that the patient should accept the help of the nurse, for them to have cooperation. Then after it, the nurse should make the patient accept that her care is only temporary. I appreciate her because, she really do understand the feelings of her patient. Because as a nurse, we must know the physical, emotional, spiritual, intellectual, and social being of a person not only his/her needs for being a sick person. Medicines are not enough for a person to have his/her recovery. It's a nice feeling that you made your patient remember you for the rest of her life.

PSYCHODYNAMIC NURSING
by: Hildegard Peplau

Well, In my own opinion i think that Hildegard Peplau's theory is very useful because it discuss about the four phases of nurse-patient relationship 1) ORIENTATION - which is very important to make sure that you/ we can become part of our patients life.
2) IDENTIFICATION - after the orientation phase, when you are already somehow involved with the life of your patient it will be easy for you and your patient to interact because you will feel comfortable even if you are not related with each other personally, you would know how they feel and know their needs.
3) EXPLOITATION - in this phase both of you will have a relationship which moves from being dependent to being independent and there will be new goals to be achieved.
4) RESOLUTION - this final phase tells how the patient earns his independence after all the work that had been done by the nurse, and that the nurse-patient relationship would have a lasting impression.

but not only the 4 phases of nurse-patient relationship but also the roles of a nurse not only as a care giver who just give care and heal the patient physically but also emotionally, spiritually socially and psychologically because if it only end as a nurse and you didn't guide them thoroughly with their spiritual needs, your role as a NURSE will be incomplete and your patient will not be totally healed.

Within reading this article. . .

I learned that in nurse-patient relationship, both the nurse and the patient must have cooperation to one another in seeking the patient’s dependency to an independent nurse. That’s why there are four-phase phenomenon involved to effectively understand how to pass every single phase and achieve the very last phase in which a nurse must teach the patient to be independent on his own and let go.

“Nurse is not a nurse alone.”
I was confused before because I thought that nurses have same obligations when they were in duty in a hospital. I thought that the responsibilities of nurses were just reading the thermometer, injections, getting the blood pressure, and arranging the dextrox. But now, I know that there are different kinds of nurses with different roles in which it can be a stranger, resource person, teaching, counseling, surrogate, and active leadership role a nurse must acquire.

The Psychodynamic Nursing really helped nurses in a way that the patient and the nurse will have the chance to relate with each other. In this theory, there is a sequence of procedures or process of how to interact with each other.
The nurse and the patient will be strangers at first and soon will know the needs of each other. The nurse must gain the trust of his/her patient and develop that trust for them to have a great relation. The nurse will be the one to teach and lead and the patient must cooperate with the interventions so that he/she will learn how to take care of him/herself. If the patient learned how to be independent, then that is the time to free him/herself from the interaction.
The theory also has a great contribution in education, research and practice. The theory really deserve to pass in simplicity, generality, empirical precision and derivable consequences because it is easy to understand, applicable to all nurses, factual, and it widened the perception of nursing.

after you had discussed peplau's theory i learned that she had been influenced by differnt people before she come up with this famous theory such as maslow, miller, pavlov, and sullivan. this theory taught us that understanding one's own behavior is to help others to overcome their difficulties and problems. as a nurse it is our primary responsibility to help our clients to surpass their disabilities physically, morally
, spiritually and emotionally.

in a nurse patient relationship,it undergoes different phases such as orientation phase where patient and nurse know and introduce each other.they start to build their trust and rapport then, identification phase will follow wherein identifying the problems are the main priority. expoitation phase-where implementation of plan care is evaluated.then, the patient aspires for new goals.

as a nurse, we act asa pcholological mother for all of our patients.

peplau's psychodynamic nursing has a great contribution in the field of nursing. we all know that having good interrelationship with others will really help alot in associating with someone. we define this as dynamic relationship between a nurse and patient, and being able to understand one's own behavior.

in my daily life i can apply her theory even im just only starting right now my journey in the field of nursing, because there is such situation that i can use her theory, for example in our school in our home, and even in other places because it is really important that we have a good interaction to others so for that we can easily associate with them.
because it is true that we encounter strangers in our everyday life and we really dont know who they are so we should understand her owns behavior so for us to be easy to have a good relationship with them.

im so thankful that hildegard peplau built her theory because without it probably there is a psychotic nurses.

The Psychodynamic Theory of Hildegard Elizabeth Peplau is really useful and applicable in all aspects of nursing profession.
It shows the importance of having a good interaction between the nurse and the patient, but in order to have this good interaction it is important to pass the four phases of relationship. I learned that the nurse must know how to interact with other people for it is her duty, but not because it is her obligation she must give her full concern to the patient in which she must consider the patient as a human who need care. I also learned the importance of working with a smile for it help the patient to lessen the pain he has encounter.
Thanks again to HILDA for her nurse-patient Relationship that is widely useful and really applicable.

Hildegard Peplau( Psychodynamic Nursing)
Peplau is the theorist who believed in the interpersonal relationship of a nurse with the patient . She believed that a person should not only act as a boss but instead have an equal relationship with its patient.
Psychodynamic Nursing
*Psychodynamic nursing describe the dynamic relationship between a nurse and a patient. It is being able to understand one’s own behavior to help others identify felt difficulties, and to apply principles of human relations to the problems that arise at all levels of experience.Peplau challenged psychiatric nursing to be successful in this kind of study and with the nurse-patient relationship. The engagement in evidence-based practice Being competence in information technology and lastly leadership in shifting the health care paradigm to community-based delivery.
According to her, their are different roles of nursing including stranger role. As a nurse first impression is always last. Meaning a nurse should be alert with her action , his/her introduction should already have a great impression to his/her patient and with that making a great relationship as well.Resource Person Role- is a role of nursing in which a nurse is an interpreter,
she/he answer questions for the patient to understand.Teaching Role
a nurse should give his patient exact and precise instruction for he/she to be more healthy.Counseling Role is also like resource person but the nurse further explain some questions and it mostly talk about the situation of the patient.Surrogate Roles, the nurse helps client clarify domains of dependence, interdependence, and independence meaning she /he help the patient to be independent with his life and for making his/her comfortable.Active Leadership Role,a nurse should have a democratic relationship with his/her patient , the nurse should be responsible enough to make his/her patient's goal to achieve.
Their are also 4 phases of inter personal relationship of a nurse.
Orientation- it's the identification of problem with the help of the nurse but in this phase still sometimes the patient still refuses help with the nurse. Identification - in which the person identifies with the nurse, meaning the patient accept the help of the nurse. Exploitation - in which the person make use of the nurse's help, this phase is the connection of a more strong bonding of the patient and nurse.Resolution - in which the nurse and his/her patient end their relationship cause the nurse frees his/her patient from their relationship as nurse-patient.
According to the NURSING METAPARADIGM:
Nurse- according to Peplau its a interpersonal relationship.
Environment-according to Peplau its the culture and the beliefs of the person.
Health- according to Peplau its the symbol of movement or the activity of a person.
Person-according to Peplau its the man.
According to Peplau theirs an Application and Importance of her Theory
PRACTICE- the use of interpersonal relationship and counseling.
EDUCATION-it give idea for psychiatric nursing to be made.
RESEARCH- new knowledge about interpersonal relationship and psychology is being discovered.
Analysis of the Theory
SIMPLICITY-Its easily understood.
GENERALITY-its applied to every environment and by every nurse.
EMPIRICAL PRECISION- its base on reality.
DERIVABLE CONSEQUENCES-It helps make the research broad.

Nowadays, a relationship exists everywhere... But for Hildegard Peplau, she focus on her theory about nurse-patient relationship, which is also known as the "therapeutic relationship. And as I observe, her theory really exists in every setting that is very significant aspect in the nursing profession, and the most essential part of the nursing care.

Through this, it builds trust, therefore forming a strong relationship between nurse and a patient.

In nurse-patient relationship, there are four phases that should be considered to accomplish their goals. Moreover, nurse may undertake different roles within the relationship to reach the satisfaction and growth.

I do believe that the purpose of relationship is not to have someone who might complete you but to have someone whom you might share your incompleteness with. Here, the nurse share and fulfill the patient's incompleteness while having learnings through their experiences.

Stress management really helps in our daily living... And although anxiety is contagious, smile is curable.

I believe that that a SMILE is such a simple gesture yet such a powerful weapon and instrument. It can touch hearts and even change lives. So I would say now that we should keep smiling! We might inspire someone with it...

Psychodynamic Nursing helps a lot in the nursing profession and even favorable for the patients. This theory has a great emphasis on the dynamic relation between nurse and the patients. For Peplau, the relation between the nurse and the patient is like a four-phase phenomenon which includes the orientation, identification, exploitation and resolution. This explains that in caring our patients, having a good communication with them will help us to have easier way of caring them and to identify also their emotional and mental needs not only their physical needs.

As a nurse, we have different and various roles to our patients. Roles that we need to accomplish so that our patients will achieve their wellness in the right time. We need to know that our patients need us while they are suffering from their illness. Because of that, we should implement the "Psychological Mothering". We should accept our patients whatever they are and we should sustain their needs in order for them to recover.

We can apply this theory on our respective professions because it is really effective. Thus, this theory elaborates the good thing about having a communication or a "professional relationship" on our patients during our duty...

Hildegard's Psychodynamic Nursing Theory helps a lot in the nurse-patient relationship. It is more on proper way of communicating with the patient in order to have a good relationship with each other. Also it helps in understanding the patient's emotional status and to be able to conquer it and have analysis upon one's behavior.

The Interpersonal relations theory is more about earning the trust of the patient and being confident with each other by knowing one another. Being able to give what is needed not only physically but also emotionally. Also a person has different well-beings that should be given concern of,which are the physical aspect, emotional aspect, mental aspect, spiritual and sociological aspect. A nurse must be able to interpret then analyze one's behavior, also to cure the patient not only focusing on the physical aspect.

For Hildegart Peplau to create her nursing theory, she obtain and link the ideas from the mentioned psychologists that shape her nursing work. Peplau's Psychodynamic Nursing discusses not only the relation between the nurse and the patient but also how to understand one's own behavior to help others identify and solve their problem.
Peplau explain the Nurse-Patient Relationship. And for me it's just like a story. At first scene, they see and get to know each other until they became close and recognize each other needs. The patient needs the help of the nurse to recover while the nurse needs the cooperation of the patient. The nurse must/should give the patients needs like counselling, answering/clarifying the patient's question, teaching and others. Then at last, they need to separate, the stranger(patient) needs to continue his journey and aim a new goal after being too dependent at the time when the nurse accepted him with a warm welcome.
Peplau's major assumption describe the nurse itself when giving nursing care to each patient makes difference on what they will learn. For example, if the nurse was moody when he/she takes care of a patient, of course the possible effect of the nurse's behavior to the patient is that he/she will think that the nurse don't want to take care of him/her without knowing that the real reason of the nurse moodiness was not the one he thinks but other reason. So the nurse should give nursing care with a nice mood, smiling face, he/she should not bring her other problem to hospital during her work for him/her also to get the cooperation of the patient. Peplau's Theory is always applicable and should practice by the nurses, doctors, or other health workers to preserve the value of life of every patient they're taking care of.

Psychodynamic Nursing by Hildegard Peplau
It is really an influential to all under the nursing profession, why? because she believes that nurse and the patient must have their proper interaction with each other not only physically but also emotionally.
NURSE-PATIENT RELATIONSHIP
(Interpersonal Relations Theory)
Orientation - because of this we could know our individual problems and could help us how to have the proper interaction with each other.
Identification - the person identifies the nurse and could help them with their needs.
Exploitation - the person make use given by the nurse. The nurse function is by filling the needs of their patient
Resolution - the person can adjust themselves of their situation and could assist each of them individually without the need of the nurse. In short the patient mus stand on their own.
NURSING ROLES
1. Stranger Role: We all know that once the patient was on the hospital they frequently doesn't knew the professionals in the hospitals just for example the nurse.Even though they do not know the Nurse they should have the interaction with each other so the patient must have their trust to the nurse. So the nurse should interact to the patient kindly to have their trust to them.
2. Resource Person Role: If the patient has a question nurses should always have the answers and we must make sure that they could understand it.
3. Teaching Role: This is when the nurse could function as a teacher that they give instruction and people can learn from them not only because of the information they learn from educational literature but also in their experiences.
4. Counseling Role: This is when the nurse advises the patient of their problems.
5. Surrogate Role: This is when the patient because of the the nurse could have their own independence and could understand the difference of dependence, independence and interdependence.
6. Active Leadership Role: Helps the patient to responsible of their health and have a good interaction with the nurse.
NURSING METAPARADIGM
Nursing- it is the interpersonal relationship.
Person- with different characteristics and must have different interaction according to their behavior.
Health- Doesn't always focus on physical wellness of the person but also on mental wellness.
Environment- must respect each culture, customs and beliefs.
Application and Importance of the Theory
PRACTICE:this can have a positive effect to the nurse-patient relationship. Could help them understand each other much better.
EDUCATION: Could help the future nurses on how will they interact to different patients with different belief, culture, etc.
Analysis of the Theory
SIMPLICITY
We could understand it simple because this theory specifies the exact situation.
GENERALITY
All the Nurse can always apply this theory in different situation.
EMPIRICAL PRECISION
Shows the truths that many situations can undergo this theory.
DERIVABLE CONSEQUENCES
Makes the perception of the nursing process more wide. Could affect all of us

Psychodynamic Nursing, Hildegard Peplau's theory as I have noticed is actually about the Nurse-patient relationship and the trust given by the patient to the nurse.

TRUST is such a big word. it is not that easy to achieve. Nurses need to gain the trust of the patient for them to work harmoniously. nurses and patients need to have one same goal to work with and later on succeed through it.

Hildegard Peplau's theory is very useful in the nursing profession. She used the term, psychodynamic nursing, to describe the dynamic relationship between a nurse and a patient.

She described four phases of this relationship: orientation, in which the person and the nurse mutually identify the person's problem; identification, in which the person identifies with the nurse, thereby accepting help; exploitation, in which the person makes use of the nurse's help; and resolution, in which the person accepts new goals and frees herself or himself from the relationship.

Today Peplau's "Interpersonal Relations in Nursing" is recognized as the first nursing text with a theoretical perspective, laying the groundwork for psychiatric nursing as a specialty. Her book, translated into nine languages, has been universally integrated into nursing education and practices throughout the world.

The work of Peplau is very much relevant to the profession I’m taking in. It gives a wide view on how nurses can help their patients through good relationship with them. I think it’s one of the best factors to help patient conquer their fears and frights.
Once you choose the nursing profession, each one should not only memorize the theories, facts as well as practices. But more importantly, they should know how to apply them to daily life situations. Most of all, they should not only give a portion of their life, but give even more than 100% of their abilities just to satisfy their patients and make them feel that life is worth living.
I just wish that I can develop my interpersonal skills so that someday, if I’m exercising my profession in the real world, I can easily respond to the needs of my patients.

This is such an interesting concept because it tells about the Nurse-Patient relationship. This allows us on how to interact with the patients well. She also conceptualized the orientation, identification, exploitation and resolution that made me realized that without these concepts, it may result errors to the patients. So nurses should be aware of the patient's behavior that can affect their situation. Nurses should also understand one's knowledge to attain our goal. and that's to gain a good relationship with our patients that may affect to their recovery.

Hildegard Peplau is an achiever personality in the field of nursing profession. The Interpersonal therapeutic Process is useful in helping psychiatric patients become receptive for therapy.
"Psychological Mothering" includes the following step:
-The patient is accepted unconditionally as a participant in a relationship that satisfies his needs.
-There is recognition of and response to the patients readiness for growth,as his initiative;
-Power in the relationships shifts to the patient,as the patient is able to delay gratification and to invest in goal achievement.
In the course of the nurse-patient relationship, the nurse assumes several roles(Role of the stranger,Role of the Resource Person,Teaching Role,Leadership Role,Surrogate Role,Counseling role) which empower and equip her in meeting the needs of the patient.

She also stated phases that will enable the nurse and patient to create a good relationship to each other, it will be a way for the nurse to implement the plan of care to the patient and lastly, help the patient to become independent. Accorrding to Peplau, there are roles of nurses that will help the patient in meeting his needs. Peplau and her Psychodynamic Nursing was a great contribution to the nursing profession. It can be applied in our daily lives. This can help nurses or even nursing students like me to understand the uniqueness of every individual, their behavior in order for us to develop the ability to cope with them.

I agree with her that the nurse-patient interaction is important go have a good relationship with the patient which can help in solving problems especially when it is related with health. I also believe that therapeutic and verbal communications between individuals, especially in the case of a nurse and the recipient of her care, can really contribute in stress management, releasing of hidden emotions and fear. This can be a means of showing sympathy and care in a way that a patient can feel that they are loved.=)

Hildegard E. Peplau's concept in her theory are just right. This is very applicable and useful not just in nursing the patient but also in our relationship with other people. Nurses' roles and responsibilities are not just a helper of the doctor, take the blood pressure or give the medicine. Their role is to be in behalf of the patient, wherein the nurse reports the situation of the patient;serves as a councelor, where the nurse listens, give advices and encourage the patient; a teacher and a resource person that he/she explains the terms the patient can't understand and also gives information and instruction to help the patient learn;a leader wherein his/her responsibility should be done. And the 4 phases in nurse relationship:orientatin, identification, exploitation and resolution should be done. This is very useful to have a healthy nurse-patient relationship. And while that, nurses should help the patient to be independent by and by.
Her theory is really great, that the nurse learn to provide a quality nursing care...Good thing a person named Hildegard Peplau was born to improve nursing....:)

Introducing Peplau’s theory resulted to establishment of different major concepts. An example of these concepts is the Nurse-Patient Relationship. Also, different phases with its own individual self were analyzed and for me, are very important in such a way that every word counts. Roles were also emphasized on what nurses can become in front of a patient. This was very helpful because nurses may be able to conceptualize and what nurses shall be with a patient especially during their first meeting. There were also assumptions that she assumed what kind of person a nurse becomes and how to do with the patient’s maturity excel. Criteria needed for it to become a nursing theory were all achieved so I believe Peplau’s theory is really good and easy to understand.

The Nurse-Patient Relationship of Hilda Elizabeth peplau is the interaction of the patient with the nurse. The nurse gives the need of the patient, and she helps the patient to her recovery.

She proposed four phases to have a good interaction with each other. First is the orientation, it happens when the nurse introduce her self, the second one is the identification where the patient accepted the nurse, and try to know more. Third is the exploitation phase, the nurse will encourage the patient in a good way to cooperate in the procedures. And the last one is the resolution wherein in the time that the patient is in a good situation, the nurse must end up the relationship she built with the patient as part of the phases.

The relationship is for everyone by smiling not giving stress to others will give you a strong relationship.

Her theory is very much understood in a way that there is the simplicity in her work, the empirical precision that tells that is had been into different studies,it is derivable consequence for it is applicable in may areas and it touches many lives, and it is general because it is a broad theory.

A nurse must to not only concentrate to the cure of the body but also to the emotional, and spiritual being of the patient .It is also said that the nurse must identify what is the problem of the patient.

A patient must develop trust to the nurse. At the end of the phases the nurse must assist the patients to be independent .The theory is simple and easily to understand, and can be apply to any situation.

She developed different phases that help the students to undergo on that phases to achieve their goals in caring a patient. I can also say that Psychodynamic nursing of Peplau is very inspiring. Especially to people like us, as a nursing students. It was inspiring in the sense that we as a nurses we should assure that we have a good relation or communication with our patient’s, to the physicians that we are helping on and also to the family of the patient that we are caring of. In this aspect the “Interpersonal relations in nursing” of Peplau enters, meaning “the communication of one person to another”, through this practice it will lead us in the promotion of better health care. I think everyone should know Peplau’s works, so we would find no difficulty in handling patients and understanding their feelings and emotions, and also to be encourage, because Peplau is a courageous woman, courageous in such a way that she left us a legacy of practical knowledge which is there is no any good or better university can teach us, to enable us survive and to continue develop about nursing. It has contributed greatly to the study of nursing. From the meaning of psychodynamic nursing which is the dynamic relationship between the nurse and the patient. Her interpersonal relation theory is also related because of the nurse-patient relationship which begins from the orientation which the nurse mutually identify the person's problem. So, in orientation the nurse will know the problem of her patient and his/her behavior. In that case of their meeting the nurse will have an idea or what she will apply to her patient. Peplau also introduced the psychological mothering which is accepting the patient's needs, unconditionally. Patients with mental disorders need the care and love unlike other normal patient, because they don't know what is happening in the world. We are so lucky because we grew in a normal life.
